负压封闭引流联合湿润烧伤膏序贯治疗压疮的临床效果

摘　　要：目的研究压疮疾病使用负压封闭引流联合湿润烧伤膏序贯治疗的临床效果。方法选取100例压疮患者,根据治疗方案不同分为对照组和观察组,各50例。对照组使用负压封闭引流治疗,观察组使用负压封闭引流联合湿润烧伤膏序贯治疗。对比两组临床疗效,治疗前后生活质量评分、睡眠质量评分、基质金属蛋白酶-9水平、基质金属蛋白酶抑制剂-1水平及创面进入红期时间、创面愈合时间、疼痛评分。结果治疗后两组的生理功能、躯体疼痛、精力、社会功能评分明显提升,且观察组的生理功能评分(25.26±2.32)分、躯体疼痛评分(9.19±0.89)分、精力评分(20.22±1.81)分、社会功能评分(8.05±0.77)分均明显高于对照组的(20.74±1.83)、(8.04±0.78)、(16.74±1.48)、(6.62±0.64)分(P<0.05);治疗后两组的基质金属蛋白酶-9、基质金属蛋白酶抑制剂-1水平均明显改善,且观察组明显低于对照组(P<0.05);观察组的临床治疗总有效率为98.00%,明显高于对照组的84.00%(P<0.05);观察组的睡眠质量评分(1.53±0.04)分、入睡时间评分(0.51±0.04)分、睡眠时间评分(0.56±0.05)分、睡眠效率评分(0.62±0.05)分、睡眠障碍评分(0.66±0.06)分、催眠药物评分(0.54±0.04)分、日间功能障碍评分(0.46±0.03)分均明显低于对照组的(2.25±0.11)、(1.18±0.10)、(1.31±0.12)、(1.26±0.11)、(1.29±0.12)、(1.19±0.10)、(1.10±0.09)分(P<0.05);观察组的创面进入红期时间(5.15±0.47)d、创面愈合时间(40.74±3.86)d均短于对照组的(11.09±0.98)、(59.57±5.65)d,视觉模拟评分法(VAS)评分(2.24±0.21)分低于对照组的(4.92±0.47)分(P<0.05)。结论压疮患者接受负压封闭引流联合湿润烧伤膏序贯治疗,患者的疼痛明显减轻,创面可以快速愈合,有效提升了患者的睡眠质量和生活质量,临床治疗效果显著。Objective To study the clinical effect of vacuum sealing drainage combined with moist exposed burn ointment in sequential treatment of pressure ulcer.Methods 100 patients with pressure ulcer were selected and divided into a control group and an observation group according to different treatment regimens,with 50 cases in each group.The control group was treated with vacuum sealing drainage,and the observation group was treated with vacuum sealing drainage combined with moist exposed burn ointment.Both groups were compared in terms of clinical efficacy,quality of life score,sleep quality score,matrix metalloproteinase-9 level,matrix metalloproteinase-1 level before and after treatment,time required for the wound to enter the red stage,wound healing time and pain score.Results After treatment,the scores of physical function,somatic pain,energy and social function in both groups were obviously improved;in the observation group,the physical function score was(25.26±2.32)points,the somatic pain score was(9.19±0.89)points,the energy score was(20.22±1.81)points,and the social function score was(8.05±0.77)points,which were higher than(20.74±1.83),(8.04±0.78),(16.74±1.48),and(6.62±0.64)points in the control group(P<0.05).The levels of matrix metalloproteinase-9 and matrix metalloproteinase-1 in both groups were obviously improved,and the observation group was obviously lower than the control group(P<0.05).The total effective rate of the observation group was 98.00%,which was significantly higher than 84.00%of the control group(P<0.05).In the observation group,the sleep quality score was(1.53±0.04)points,the sleep latency score was(0.51±0.04)points,the sleep time score was(0.56±0.05)points,the sleep efficiency score was(0.62±0.05)points,the sleep disorder score was(0.66±0.06)points,the hypnotic drug score was(0.54±0.04)points,and the daytime dysfunction score was(0.46±0.03)points,which were obviously lower than(2.25±0.11),(1.18±0.10),(1.31±0.12),(1.26±0.11),(1.29±0.12),(1.19±0.10),and(1.10±0.09)points
