苦参汤加减治疗肛瘘术后继发性肛周湿疹的效果

摘　　要：目的观察苦参汤加减治疗肛瘘术后继发性肛周湿疹的效果。方法以肛瘘术后继发性肛周湿疹患者90例作为观察对象,电脑便利抽样将患者分为两组,分别为对照组和研究组,各45例。对照组患者给予曲安奈德益康唑乳膏涂抹于患处,研究组患者在对照组患者的基础上给予苦参汤加减熏洗治疗,均连续治疗2周。比较两组患者临床疗效、治疗前后血清炎性因子水平、治疗前后临床症状积分及肛门功能评分。结果研究组患者临床治疗总有效率为95.56%,同比于对照组的82.22%明显更高(P<0.05);研究组患者白细胞介素-4(IL-4)(38.57±7.84)pg/ml、干扰素-γ(IFN-γ)(3.61±1.07)pg/ml测定值同比于对照组患者的(53.42±8.90)、(5.43±1.18)pg/ml明显更低(P<0.05);研究组患者临床症状积分(4.53±0.97)分同比于对照组患者的(7.07±1.84)分明显更低(P<0.05),两组患者肛门功能评分无差异(P>0.05)。结论苦参汤加减应用于肛瘘术后继发性肛周湿疹患者疗效确切,有助于减轻患者的临床症状,降低炎性因子水平,具有临床推广应用价值。Objective To observe the effect of modified Kushen decoction on secondary perianal eczema after anal fistula surgery.Methods 90 patients with secondary perianal eczema after anal fistula were selected as observation subjects.The patients were divided into a control group and a study group by computer assisted sampling method,with 45 cases in each group.The control group was treated with topical application of triamcinolone acetonide and econazole nitrate cream,while the study group was treated with modified Kushen decoction fumigation on the basis of the control group.All patients were treated continuously for 2 weeks.The clinical efficacy,serum inflammatory factor level,clinical symptom score and anal function score before and after treatment were compared between two groups.Results The total effective rate of clinical treatment in the study group was 95.56%,which was significantly higher than 82.22%in the control group(P<0.05).The measured values of interleukin-4(IL-4)and interferon-γ(IFN-γ)in the study group were(38.57±7.84)and(3.61±1.07)pg/ml,which were significantly lower than(53.42±8.90)and(5.43±1.18)pg/ml in the control group(P<0.05).The symptom score of(4.53±0.97)points in the study group was significantly lower than(7.07±1.84)points in the control group(P<0.05),and there was no difference in anal function score between the two groups(P>0.05).Conclusion Modified Kushen decoction has a definite therapeutic effect on patients with secondary perianal eczema after anal fistula surgery,which helps to alleviate clinical symptoms,reduce inflammatory factor levels of patients.It has clinical promotion and application value.
