摘　　要：目的观察胸腺肽α1联合乌司他丁免疫调节治疗脓毒症(SS)的临床效果。方法采用摸球法将110例SS患者分为两组,每组55例。对照组采用拯救脓毒症战役(SSC)经典治疗,观察组采用胸腺肽α1联合乌司他丁免疫调节治疗,比较两组患者的临床疗效、炎症因子、健康状况调查表(SF-36)评分、血气指标。结果治疗后,观察组的有效率98.18%,高于对照组的81.82%,差异有统计学意义(P<0.05)。治疗10 d后,两组血气指标水平相比,观察组PaO_(2)、SaO_(2)的水平均高于对照组,PaCO_(2)水平低于对照组(P<0.05);治疗10 d后,观察组hs-CRP、PCT、IL-6、TNF-α的水平均低于对照组(P<0.05);治疗10 d后,观察组CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)的水平均低于对照组(P<0.05);观察组SF-36评分高于对照组(P<0.05)。结论应用胸腺肽α1联合乌司他丁治疗SS效果确切,能快速改善患者血气指标与免疫功能,控制炎症反应,利于预后。Objective To analyze the clinical effects of thymosinα1 combined ulinastatin immunomodulatory therapy in the treatment of sepsis(SS).Methods 110 SS patients were divided into 2 groups using the touch ball method,with 55 cases in each group.The control group were received classic treatment in the Battle to Save Sepsis(SSC),while the observation group were received thymosinα1.The clinical efficacy,inflammatory factors,the mos item short from healthy survy(SF-36)score,and blood gas indicators were compared between two groups of patients.Result After treatment,the total effective rate of the observation group was 98.18%,which was higher than the control group′s 81.82%,and the difference was statistically significant(P<0.05).After 10 d of treatment,the levels of blood gas indicators in the observation group were higher than those in the control group,and the levels of PaO_(2) and SaO_(2) were lower than those in the control group(P<0.05).After 10 d of treatment,the levels of hs-CRP,PCT,IL-6 and TNF-αin the observation group were lower than those in the control group(P<0.05).After 10 d of treatment,the levels of CD3~^(+),CD4~^(+),CD8~^(+),CD4~^(+)/CD8~^(+)in the observation group were lower than those in the control group(P<0.05).The SF-36 score of the observation group was higher than that of the control group(P<0.05).Conclusion The application of thymosinα1 combined with ulinastatin in the treatment of SS patients has a definite effect,which can quickly improve the patient′s blood gas indicators and immune function,control inflammatory reactions,and facilitate prognosis.
