摘　　要：目的探讨宫腔镜子宫内膜形态学评分(HEMS)联合血清血管生成素2(Ang-2)对不孕合并慢性子宫内膜炎的诊断预测价值。方法选取2017年1月~2021年12月收治的80例不孕合并慢性子宫内膜炎患者作为研究组,另选取同期80名健康体检志愿者作为对照组,比较两组受试者HEMS以及血清Ang-2表达水平之间的差异。探讨HEMS联合血清Ang-2表达水平对不孕合并慢性子宫内膜炎的诊断效能。结果研究组患者宫腔内出血情况、内膜血管扩张情况、内膜息肉、宫腔黏液、内膜肉样增生、微孔情况评分以及血清Ang-2表达水平均高于对照组(P<0.01),且以上因素均是造成患者不孕合并慢性子宫内膜炎的独立危险因素(P<0.01)。HEMS联合血清Ang-2表达水平诊断不孕合并慢性子宫内膜炎的准确率、灵敏度、特异度等均高于二者单独诊断(P<0.01)。受试者工作特征(ROC)曲线分析显示:HEMS联合血清Ang-2表达水平预测不孕合并慢性子宫内膜炎的曲线下面积(AUC)高于二者单独预测的AUC。结论HEMS联合血清Ang-2对不孕合并慢性子宫内膜炎的诊断具有积极意义,可显著提升患者的诊断效能。Objective To explore the predictive value of hysteroscopic endometrial morphology score(HEMS)combined with angiopoietin-2(Ang-2)expression level for the diagnosis of infertility complicated by chronic endometritis(ICCE).Methods Eighty ICCE patients treated from Jan.2017to Dec.2021 were selected as research group and 80 health examination volunteers as controls in the same period,differences of HEMS and Ang-2 expression levels between the two groups compared.Diagnostic efficacy of HEMS and Ang-2 expression levels for the ICCE was explored.Results Such indexes were higher in research than control group as intrauterine bleeding,endometrial vascular dilation,endometrial polyps,intrauterine mucus,endometrial hyperplasia,and microperforations score as well as Ang-2 expression level(P<0.01)and above-mentioned factors were all independent risk factors causing the ICCE(P<0.01).Accuracy,sensitivity and speciality of HEMS combined with Ang-2 expression level in the diagnosis of the ICCE were higher than those of separate HEMS or Ang-2expression level(P<0.01).Receiver operating characteristic(ROC)curve showed that the area under the ROC curve of HEMS combined with Ang-2 expression level predicting the ICCE was higher than that predicted by separate HEMS or Ang-2 expression level.Conclusion The combination of HEMS and Ang-2 expression level has a positive significance in the diagnosis of the ICCE and can significantly improve diagnostic efficacy for patients.
