摘　　要：目的:探讨A型肉毒毒素联合真皮内超减张缝合治疗病理性瘢痕的效果。方法:回顾性分析赣州市人民医院2021年3月—2023年3月收治的62例病理性瘢痕患者的临床资料,根据不同治疗方法分为两组,各31例;其中对照组采用真皮内超减张缝合,试验组采用A型肉毒毒素联合真皮内超减张缝合。比较两组温哥华瘢痕评价量表(VSS)评分、观测者瘢痕评价量表(OSAS)评分、瘢痕宽度、瘢痕厚度、切口不良反应发生情况及复发情况。结果:术前,两组OSAS评分、VSS评分、瘢痕宽度、瘢痕厚度比较,差异均无统计学意义(P>0.05);术后6个月,两组OSAS评分、VSS评分、瘢痕宽度、瘢痕厚度均低于术前,且试验组均低于对照组,差异均有统计学意义(P<0.05)。两组术后切口不良反应发生率比较,差异无统计学意义(P>0.05);试验组复发率为3.23%,低于对照组的19.35%,差异有统计学意义(P<0.05)。结论:A型肉毒毒素联合真皮内超减张缝合可有效促进病理性瘢痕患者切口愈合,提高切口美观度,减轻瘢痕形成程度,降低复发率。Objective:To investigate the effect of Botulinum Toxin Type A combined with intradermal hypertension suture in the treatment of pathological scar.Method:The clinical data of 62 patients with pathological scar admitted to Ganzhou People's Hospital from March 2021 to March 2023 were retrospectively analyzed.They were divided into two groups according to different treatment methods,with 31 cases in each group.The control group underwent intradermal hypertension suture,and the experimental group underwent Botulinum Toxin Type A combined with intradermal hypertension suture.Vancouver scar rating scale(VSS)score,observer scar assessment scale(OSAS)score,scar width,scar thickness,incidence of incision adverse reactions and recurrence were compared between the two groups.Result:Before surgery,there were no statistical significance in OSAS score,VSS score,scar width and scar thickness between the two groups(P>0.05).At 6 months after surgery,OSAS score,VSS score,scar width and scar thickness between the two groups were lower than those before surgery,and those in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative incision adverse reactions between the two groups(P>0.05).The recurrence rate of experimental group was 3.23%,which was lower than 19.35%of control group,the difference was statistically significant(P<0.05).Conclusion:Botulinum Toxin Type A combined with intradermal hypertension suture can effectively promote the wound healing of patients with pathological scars,improve the aesthetic of the incision,reduce the degree of scar formation,and reduce the recurrence rate.
