自拟活血润肠方联合地奥司明片治疗混合痔术后水肿的临床效果和安全性

摘　　要：目的:探讨自拟活血润肠方联合地奥司明片治疗混合痔术后水肿的临床效果和安全性。方法:按随机数字表法将2023年1—10月景德镇市中医医院肛肠科诊治的80例混合痔术后水肿患者分为对照组和治疗组,各40例。对照组采用口服地奥司明片治疗,治疗组采用自拟活血润肠方联合地奥司明片治疗,均连续治疗14 d。比较两组水肿体积、水肿消退时间、肛门疼痛、负性情绪、服用止痛药情况和并发症发生情况。结果:治疗组术后7、14 d水肿体积均小于对照组,水肿消退时间早于对照组,差异均有统计学意义(P<0.05)。治疗组术后7、14 d视觉模拟评分法(VAS)评分均低于对照组,差异均有统计学意义(P<0.05)。治疗组术后7、14 d后焦虑自评量表(SAS)评分均低于对照组,差异均有统计学意义(P<0.05)。治疗组服用止痛药比例低于对照组,并发症总发生率(7.50%)低于对照组(30.00%),差异均有统计学意义(P<0.05)。结论:混合痔术后水肿患者采用自拟活血润肠方+地奥司明片联合治疗,能够减轻术后负性情绪、肛门水肿和疼痛感。Objective:To investigate clinical effect and safety of self-made Huoxue Runchang Prescription combined with Diosming Tablets in the treatment of postoperative edema of mixed hemorrhoid.Method:A total of 80 patients with postoperative edema of mixed hemorrhoid who were diagnosed and treated in Department of Proctology,Jingdezhen Hospital of Traditional Chinese Medicine from January to October 2023 were divided into control group and treatment group according to the random number table method,with 40 cases in each group.The control group was treated with Diosming Tablets,and the treatment group was treated with self-made Huoxue Runchang Prescription combined with Diosming Tablets,two groups were treated continuously for 14 days.Edema volume,edema resolution time,anal pain,negative emotion,analgesic administration and complications were compared between two groups.Result:After 7 and 14 days of surgery,the edema volume of the treatment group were smaller than those of the control group,and the edema resolution time was earlier than that of the control group,the differences were statistically significant(P<0.05).After 7 and 14 days of surgery,the scores of visual analogue scale(VAS)in the treatment group were lower than those in the control group,the differences were statistically significant(P<0.05).After 7 and 14 days of surgery,the scores of self-rating anxiety scale(SAS)in the treatment group were lower than those in the control group,the differences were statistically significant(P<0.05).The proportion of analgesic administration in the treatment group was lower than that in the control group,and the total complication rate(7.50%)in the treatment group was lower than that in the control group(30.00%),the differences were statistically significant(P<0.05).Conclusion:The combination of self-made Huoxue Runchang Prescription+Diosming Tablets in the treatment of patients with postoperative edema of mixed hemorrhoid can relieve postoperative negative emotion,anal edema and pain.
