血清FIB及尿RBP、NAG、TRF与紫癜性肾炎患儿肾脏病理分级的相关性

摘　　要：目的探讨血清纤维蛋白原(FIB)及尿视黄醇结合蛋白(RBP)、尿N⁃乙酰⁃β⁃D氨基葡萄糖苷酶(NAG)、转铁蛋白(TRF)与紫癜性肾炎(HSPN)患儿肾脏病理分级的关系。方法选取2021年1月至2023年5月安徽省儿童医院收治的114例HSPN患儿作为研究组,按病理等级分为Ⅱ级63例、Ⅲ级51例;另选取同期住院无肾脏损伤的过敏性紫癜患儿57例作为对照组。对比两组和研究组亚组血清FIB及尿RBP、NAG、TRF水平,分析血清FIB及尿RBP、NAG、TRF与肾脏病理分级的关系及对紫癜性肾炎的预测价值。结果研究组血清FIB、RBP、NAG、TRF水平高于对照组,差异有统计学意义(t=3.160、11.001、2.882、4.168,P<0.05)。Ⅲ级组血清FIB、RBP、NAG、TRF水平高于Ⅱ级组,差异有统计学意义(t=3.995、10.123、4.324、2.893,P<0.05)。血清FIB、RBP、NAG与HSPN肾脏病理分级均呈正相关(r=0.360、0.666、0.400、0.239,P<0.05)。ROC曲线分析显示,血清FIB及尿RBP、NAG、TRF联合检测预测紫癜性肾炎的AUC为0.930,高于各指标单独检测。结论血清FIB及尿RBP、NAG、TRF水平与HSPN患儿病理分级密切相关,联合检测能作为判断HSPN的有效指标。Objective To investigate the relationship between serum fibrinogen(FIB),urinary retinol⁃binding protein(RBP),urinary n⁃acetyl⁃β⁃D glucosaminase(NAG),transferrin(TRF)and renal pathological grade in children with purpura nephritis(HSPN).Methods A total of 114 children with HSPN admitted to Anhui Children's Hospital from January 2021 to May 2023 were selected as the study group,and divided into 63 cases of gradeⅡand 51 cases of gradeⅢaccording to pathological grade.In addition,57 hospitalized children with anaphylactoid purpura without renal injury were selected as the control group.The levels of serum FIB,urine RBP,NAG and TRF were compared between the two groups.The study group was also analyzed for the relationship between serum FIB,urine RBP,NAG and TRF and renal pathological grade.The predictive value of serum FIB and urine RBP was analyzed.Results The levels of serum FIB,RBP,NAG and TRF in the study group were higher than those in the control group(t value:3.160,11.001,2.882 and 4.168,respectively,P<0.05).The levels of serum FIB,RBP,NAG and TRF in gradeⅢgroup were higher than those in gradeⅡgroup(t value:3.995,10.123,4.324,2.893,P<0.05).Serum FIB,RBP and NAG were positively correlated with renal pathological grade(r value:3.995,10.123,4.324,2.893,P<0.05).ROC curve analysis showed that the AUC of serum FIB combined with urine RBP,NAG and TRF to predict purpura nephritis was 0.930,which was higher than that of each indicator alone.Conclusion Serum FIB,urine RBP,NAG and TRF levels are closely related to the pathological grade of HSPN children,and combined detection can be used as an effective index to judge HSPN.
