糖皮质激素在儿童结核病治疗中的合理应用专家共识

摘　　要：糖皮质激素具有抗炎、抗过敏和免疫抑制等药理作用,能够减轻机体对结核分枝杆菌变态反应所致的免疫损伤,但糖皮质激素不适当的应用可诱发感染或使结核病灶扩散、恶化,严格掌握其适应证及规范使用极为重要。基于此,中国防痨协会、中华医学会结核病学分会儿童结核病专业委员会、中国研究型医院学会结核病学专业委员会、中国医药教育协会儿科专业委员会、云南省防痨协会儿童结核病专业分会共同组织多学科专家共同制定本专家共识。主要内容涉及糖皮质激素应用范围、在儿童结核病辅助治疗的应用原则和注意事项、推荐其辅助治疗不同结核病类型的意见及说明以及在某些特定情况下推荐使用的结核病类型及说明。共识旨在规范糖皮质激素在辅助治疗儿童结核病中的合理应用。Glucocorticoids have pharmacological effects known as anti-inflammatory,anti-allergic,and immunosuppressive effects,which can alleviate immune damage caused by Mycobacterium tuberculosis.However,inappropriate use of glucocorticoid can induce worsen of the diseases or dissemination of infection.So,the use of glucocorticoids in tuberculosis should have strict standards and indications.Consequently,this consensus is formulated by Chinese Antituberculosis Association,Pediatric Tuberculosis Committee,the Society of Tuberculosis,Chinese Medical Association,Tuberculosis Committee of Chinese Research Hospital Association,China Medicine Education Association Committee on Pediatrics,Pediatric Tuberculosis Committee of Yunnan Antituberculosis Association under the cooperation of multiple disciplines,and it includes the recommended principles for the glucocorticoids application and precautions of glucocorticoid in the adjuvant treatment of tuberculosis in children,the opinions and descriptions of recommendation for the use of glucocorticoids adjuvant therapy in the common categories and the specific conditions of tuberculosis diseases.The consensus concludes evidence of glucocorticoid adjuvant therapy in childhood tuberculosis,and targets to standardize the use of glucocorticoids for children with tuberculosis.
