肺静脉隔离术对阵发性房颤患者心率减速力的影响

摘　　要：目的探究肺静脉隔离术对阵发性房颤患者心率减速力(DC)的影响。方法回顾性分析2017年6月至2021年6月运城第一医院收治的106例阵发性房颤患者的临床资料。所有入选患者均行肺静脉隔离术治疗。给予患者24 h动态心电图与心脏超声心动图检查,对比心率加速力(AC)、DC;使用心脏磁共振成像诊断,检测左心房厚壁,包括A节段、B节段。对比所有患者术前及术后即刻的AC、DC、左心房厚壁。随访3个月,详细统计复发情况并分组,对比复发组、未复发组患者的AC、DC、左心房厚壁。结果术后即刻及术后3个月,阵发性房颤患者的AC绝对值、DC值、A节段左心房厚壁、B节段左心房厚壁均低于术前,且术后3个月的AC绝对值、DC值均高于术后即刻(P<0.05)。术后随访3个月,房颤复发31例(复发组),未复发75例(未复发组)。术前,复发组与未复发组AC绝对值、DC值、A节段左心房厚壁、B节段左心房厚壁比较差异未见统计学意义(P>0.05);术后即刻及术后3个月,未复发组AC绝对值、DC值、A节段左心房厚壁、B节段左心房厚壁均低于复发组(P均<0.05)。结论肺静脉隔离术会促使阵发性房颤患者的心率减速力、左心房厚壁下降;与术后3个月复发患者相比,未复发患者的心率减速力恢复更快。ObjectiveTo investigate the effect of pulmonary vein isolation on deceleration capacity of heart rate(DC)in patients with paroxysmal atrial fibrillation.MethodsThe clinical data of 106 patients with paroxysmal atrial fibrillation treated in Yuncheng First Hospital from June 2017 to June 2021 were retrospectively analyzed.All selected patients were treated by pulmonary vein isolation.And they were given 24-hour ambulatory electrocardiogram and echocardiography examinations.The acceleration capacity of heart rate(AC)and DC of them were compared.Cardiac magnetic resonance imaging was used to detect the thick wall of the left atrium,including the segment A and segment B.The preoperative and immediately postoperative value of AC,DC and left atrial thick wall were compared.All patients were followed up for 3 months to record the recurrence in detail and grouped.The AC,DC,and left atrial thick wall were compared between the recurrence group and non-recurrence group.ResultsImmediately and 3 months after surgery,the absolute value of AC,DC value,and left atrial thick wall at segment A and segment B in patients with paroxysmal atrial fibrillation were all lower than those before surgery(P<0.05);moreover,the absolute value of AC and DC value 3 months after surgery were higher than those immediately after surgery(P<0.05).During 3-month follow-up after surgery,31 cases had recurrence of fibrillation(reccurrence group),and 75 had no recurrence(non-recurrence group).Before operation,there was no significant difference in absolute value of AC,DC value and left atrial thick wall at segment A and left atrial thick wall at segment B between the recurrence group and the non-recurrence group(P>0.05).Immediately and 3 months after surgery,the absolute value of AC,DC value,left atrial thick wall at segment A and left atrial thick wall at segment B in the non-recurrence group were lower than those in the recurrence group(all P<0.05).ConclusionsPulmonary vein isolation can decrease DC and left atrial thick wall in patients with paroxysma
