彩色多普勒超声评估老年2型糖尿病患者下肢血管病变和血管动力学改变的价值

摘　　要：目的探讨彩色多普勒超声(CDUS)评估老年2型糖尿病(T2DM)患者下肢血管病变和血流动力学改变的价值。方法抽取2021年9月至2023年5月新乡医学院第一附属收治的72例老年T2DM患者作为T2DM组,并以1∶1配比选取同期72例健康体检者作为健康组。两组均进行CDUS检查,比较两组下肢血管病变程度、血流动力学指标[血流峰值流速(VP)、搏动指数(PI)]、血管内径及下肢动脉粥样硬化斑块检出率。结果T2DM组下肢血管病变程度较健康组严重(P<0.05)。T2DM组腘动脉、股动脉、足背动脉VP低于健康组(P<0.05),PI高于健康组(P<0.05)。T2DM组股总动脉、股浅动脉、股深动脉、腘动脉、足背动脉血管内径小于健康组(P<0.05)。T2DM组下肢动脉粥样硬化斑块检出率(90.28%,65/72)高于健康组(25.00%,18/72),P<0.05。结论CDUS应用于老年T2DM患者中,能诊断下肢血管病变,反映双下肢的血流动力学改变,检出下肢动脉粥样硬化斑块。ObjectiveTo investigate the value of color Doppler ultrasound(CDUS)in evaluating vascular lesions and changes of vascular hemodynamics of lower extremities in elderly patients with type 2 diabetes mellitus(T2DM).MethodsA total of 72 elderly patients with T2DM admitted to the First Affiliated Hospital of Xinxiang Medical University from September 2021 to May 2023 were selected as the T2DM group,and 72 healthy patients hospitalized in the same period were selected as the healthy group at a ratio of 1∶1.CDUS was performed on both groups.The degree of vascular lesions and hemodynamic indexes in lower extremities,including peak velocity of flow(VP)and pulsatility index(PI),vascular inner diameter and detection rate of atherosclerotic plaques were compared between the two groups.ResultsThe degree of vascular lesions in lower extremities of the T2DM group was more severe than that of the healthy group(P<0.05).The VP of popliteal artery,femoral artery and dorsalis pedis artery in the T2DM group were lower than those in the healthy group(P<0.05),moreover,the PI of popliteal artery,femoral artery and dorsalis pedis artery in the T2DM group were higher than those in the healthy group(P<0.05).The inner diameters of the common femoral artery,superficial femoral artery,deep femoral artery,popliteal artery and dorsalis pedis artery in the T2DM group were lower than those in the healthy group(P<0.05).The detection rate of atherosclerotic plaque in the lower extremities of the T2DM group(90.28%,65/72)was higher than that of the healthy group(25.00%,18/72),P<0.05.ConclusionsCDUS can diagnose vascular lesions in the lower extremities,reflect the hemodynamic changes of the lower extremities,and detect atherosclerotic plaques in the lower extremities of elderly patients with T2DM.
