成人水痘带状疱疹病毒引起严重肺部感染1例

摘　　要：目的探讨新冠流行背景下成年人感染水痘带状疱疹病毒的临床表现及诊疗手段。方法回顾性分析2023年2月入住本院的1例成年人感染水痘带状疱疹病毒的诊治全过程。结果患者经宏基因组下一代测序技术确诊后,予以阿昔洛韦抗病毒、丙种球蛋白提高免疫,气管插管呼吸机辅助呼吸,抗细菌感染等综合治疗手段后,于第20天顺利康复出院。结论同时对于发热伴皮疹,免疫功能低下患者应考虑水痘带状疱疹病毒感染可能,对于肺部感染不明确患者建议尽早行灌洗液宏基因组下一代测序检查明确治病病原体,确诊后尽早使用抗病毒药物。Objective e To investigate the clinical manifestations and diagnostic approaches for adult infection with Varicella-zoster virus(VZV)in the context of the COVID-19 pandemic.Methods A retrospective analysis was conducted on the diagnosis and treatment process of one adult patient infected with VZV,who was admitted to our hospital in February 2023.Results After the patient was confirmed by metagenomic next-generation sequencing(mNGS),a comprehensive treatment ap-proach was initiated,including antiviral therapy with Acyclovir,immunoglobulin infusion to boost im-munity,assisted ventilation through endotracheal intubation,and management of bacterial infections.The patient successfully recovered and was discharged on the 2oth day.Conclusions In cases of fever with rash,consideration should be given to the possibility of Varicella-zoster virus infection,especially in immunocompromised patients.For patients with unclear pulmonary infections,early mNGS analy-sis of lavage fluid is recommended to identify the causative pathogen,followed by prompt administra-tion of antiviral medications upon confirmation of diagnosis.
