摘  要:目的探讨宏基因组二代测序(metagenomic next-generation sequencing,mNGS)在不明原因皮肤感染患者病原体检测中的作用。方法患者女,68岁,左膝红肿结节伴疼痛4个月,破溃20 d,提取患者病变皮肤组织样本进行宏基因组二代测序,对样本中微生物核酸序列进行分析,与病原数据库比对,鉴定病原微生物。结果mNGS检测到龟分枝杆菌(28个特异序列),并由PCR检测及Sanger测序验证,说明该皮肤组织感染的致病微生物为龟分枝杆菌。根据患者的临床症状,最终诊断为皮肤龟分枝杆菌感染。结论mNGS可辅助诊断少见病原微生物感染,为临床治疗提供方向。Objective To explore the role of metagenomic next-generation sequencing(mNGS)in pathogen detection of patients with skin infections of unknown origin.Methods A 68-year-old female patient presented with red,swollen nodules and pain in the left knee for 4 months,and ulceration for 20 days.mNGS was performed on the patient′s skin lesion samples,and the microbial nucleic acid sequences in the samples were analyzed and compared with the pathogen database to identify pathogenic microorganisms.Results mNGS detected Mycobacterium chelonae(28 specific sequences),which was verified by PCR detection and Sanger sequencing,indicating that the pathogenic microorganism causing the skin tissue infection was Mycobacterium chelonae.Based on the patient′s clinical symptoms,the final diagnosis was Mycobacterium chelonae skin infection.Conclusion mNGS can assist in the diagnosis of rare pathogenic microorganism infections and provide directions for clinical treatment.
