摘  要:目的探讨3D-Slicer+Sina增强现实技术对脑室外引流术中穿刺精准度和安全性的影响。方法采用回顾性病例对照研究。纳入广东医科大学附属医院神经外科2022年1月至2023年12月收治的行脑室外引流手术患者74例,按手术方式的不同分为观察组30例和对照组44例。观察组在3D-Slicer+Sina定位下行脑室外引流手术,对照组在传统CT定位下行脑室外引流手术。比较两组穿刺靶点偏移距离、穿刺次数、首次穿刺成功率、手术时长、术后并发症发生率、住院天数和住院费用等情况。结果观察组和对照组的穿刺例数分别为30例和44例,穿刺部位数分别为53个和72个,穿刺靶点偏移距离分别为(9.82±4.76)mm vs.(16.30±8.81)mm(P<0.001),穿刺次数为1(1,1)vs.1(1,1.75)(P=0.026),首次穿刺成功率为90.6%(48/53)vs.73.6%(53/72)(P=0.017),手术时长分别为85.00(64.75,107.50)min vs.70.00(55.00,90.00)min(P=0.040),术后并发症发生率为16.7%(5/30)vs.38.6%(17/44)(P=0.042),观察组均有明显优势。但两组住院天数和住院费用比较无统计学差异(P>0.05)。结论3D-Slicer+Sina辅助脑室外引流术可以提高脑室穿刺精准度和首次穿刺成功率,降低穿刺次数和术后并发症发生率,值得在临床上推广应用。Objective To explore the puncture accuracy and safety of 3D-Slicer+Sina augmented reality technology in external ventricular drainage(EVD).Methods Retrospective case control study was carried out in 74 eligible patients undergoing EVD in the Neurosurgery Department of Affiliated Hospital of Guangdong Medical University from January 2022 to December 2023.They were divided into an observation group(30 cases)and a control group(44 cases)according to different EVD methods.The observation group underwent ventricular drainage surgery using 3D-Slicer+Sina software positioning,while the control group underwent ventricular drainage surgery using traditional CT positioning.The deviation distance of puncture targets,the number of punctures,the success rate of the first puncture,length of hospital stay,incidence of postoperative complications,length of hospital stay,and hospitalization costs were compared between the two groups.Results In the observation group and the control group,the number of punctured patients was 30 and 44,and the number of puncture sites was 53 and 72 respectively,with the deviation distance of(9.82±4.76)mm vs.(16.30±8.81)mm(P<0.001),the number of punctures of 1(1,1)vs.1(1,1.75)(P=0.026),the success rate of the first puncture of 90.6%(48/53)vs.73.6%(53/72)(P=0.017),the surgical duration of 85.00(64.75,107.50)min vs.70.00(55.00,90.00)min(P=0.040),and the incidence of postoperative complications of 16.7%(5/30)vs.38.6%(17/44)(P=0.042).The observation group showed significant advantages.However,there was no significant difference in the length of hospital stay and hospitalization costs between the two groups(P>0.05).Conclusion 3D-Slicer+Sina software assisted extraventricular drainage can improve the accuracy of ventricular puncture and the success rate of the first puncture,reduce the number of punctures and the incidence of postoperative complications,which illustrates worthy of promoting and applying.
