摘　　要：目的探究他达那非片降阶梯治疗应用于慢性前列腺炎勃起功能障碍中的效果。方法随机选取2021年9月—2023年9月江阴市人民医院泌尿外科收治的70例慢性前列腺炎合并勃起功能障碍患者为研究对象,根据不同治疗方法分为参照组(35例,常规治疗)和研究组(35例,他达那非片降阶梯治疗)。比较两组的治疗有效率、前列腺炎症状指数、国际勃起功能指数-5、勃起硬度、前列腺小体外泄蛋白水平、阴茎血流动力学指标。结果研究组治疗总有效率为97.14%(34/35),高于参照组的82.86%(29/35),差异有统计学意义(χ^(2)=3.968,P=0.046)。治疗后,研究组前列腺炎症状指数低于参照组,国际勃起功能指数-5与勃起硬度评分高参照组,差异有统计学意义(P均<0.05)。研究组前列腺小体外泄蛋白水平低于参照组,差异有统计学意义(P<0.05)。研究组阻力指数、舒张末期流速高于参照组,收缩期峰值流速低于参照组,差异有统计学意义(P均<0.05)。结论应用他达那非片降阶梯治疗慢性前列腺炎伴勃起功能障碍,具有明显的临床应用价值。Objective To explore the effect of tadalafil tablets de-escalation therapy on erectile dysfunction in chronic prostatitis.Methods A total of 70 patients with chronic prostatitis complicated with erectile dysfunction admitted to the Department of Urology,Jiangyin People's Hospital from September 2021 to September 2023 were randomly selected as the research objects.According to different treatment methods,they were divided into the reference group(35 cases,routine treatment)and the study group(35 cases,tadalafil tablets de-escalation treatment).The effective rate of treatment,prostatitis symptom index,international index of erectile function-5,erectile hardness,prostatic corpuscle leakage protein level and penile hemodynamics were compared between the two groups.Results The total effective rate of treatment in the study group was 97.14%(34/35),which was higher than 82.86%(29/25)in the reference group,and the difference was statistically significant(χ^(2)=3.968,P=0.046).After treatment,the prostatitis symptom index of the study group was lower than that of the reference group,and the international index of erectile function-5 and erectile hardness score were higher than those of the reference group,the differences were statistically significant(all P<0.05).The level of prostatic corpuscle leakage protein in the study group was lower than that in the reference group,and the difference was statistically significant(P<0.05).The resistance index and end-diastolic velocity of the study group were higher than those of the reference group,and the peak systolic velocity was lower than that of the reference group,the differences were statistically significant(all P<0.05).Conclusion The application of tadalafil tablets in the treatment of chronic prostatitis with erectile dysfunction has obvious clinical application value.
