非大疱性嗜中性红斑狼疮1例

摘　　要：报告1例非大疱性嗜中性红斑狼疮。患者男,57岁,躯干、四肢反复出现红色丘疹、斑块伴瘙痒10余年。皮肤科检查:躯干、四肢散在分布荨麻疹样丘疹、斑块,部分斑块皮损边缘有细薄鳞屑。实验室检查抗核抗体1∶320阳性(颗粒型),抗SSA抗体(++),抗SSB抗体(+++),Ro-52抗体(+++),抗中性粒细胞胞质抗体369.09 RU/ml(0~20 RU/ml)。背部皮损组织病理检查提示表皮网篮状角化过度,基底细胞液化变性,真皮浅层可见较多中性粒细胞浸润,伴有核碎裂,浅、中层血管周围较多淋巴细胞浸润,间质内可见大量黏蛋白沉积。诊断:非大疱性嗜中性红斑狼疮。To report a case of nonbullous neutrophilic lupus erythematosus.A 57-year-old male patient presented with recurrent erythematous papules and plaques on the trunk and extremities accompanied by pruritus for over 10 years.Skin examination revealed scattered urticaria-like papules and plaques on the trunk and limbs,with thin scales on the edge of some lesions.Laboratory tests showed positive antinuclear antibodies at a titer of 1:320(granular type),anti-Sj?gren syndrome A(SSA)antibodies(++),anti-SSB antibodies(+++),anti-Ro-52 antibodies(+++),and anti-neutrophil cytoplasmic antibodies at a level of 369.09 RU/ml(reference range:0-20 RU/ml).Histopathological analysis of the skin lesions on the back revealed epidermal hyperkeratosis in a basket-like shape,liquefaction degeneration of basal cells,infiltration of numerous neutrophils in the superficial dermis with karyorrhexis,perivascular infiltration of lymphocytes in the superficial and middle dermis,and extensive extracellular mucin deposition throughout the dermis.Finally,the patient was diagnosed with nonbullous neutrophilic lupus erythematosus.
