运动链理论在功能性踝关节不稳康复患者中的应用研究

摘　　要：功能性踝关节不稳(functional ankle instability,FAI),即在踝关节损伤后关节内的神经肌肉主观感知不稳状态。常见于在日常生活和体育运动中,FAI踝关节周围韧带、关节囊、肌肉、肌腱等软组织出现不同程度的损伤,本体感受器受损,关节主观表现和随意运动发生异常。踝关节不稳会导致同侧下肢肌肉募集能力减退、反应时间延长、踝关节动态稳定性下降,导致踝关节本体感觉、周围肌力、平衡功能等神经肌肉控制障碍。FAI的治疗以康复训练为主,以恢复踝关节功能为重点。当前,运动链理论逐步应用于肌骨系统疾病康复中,基于运动链理论的整体性、传导性及其生物力学效应,更加注重对肌骨系统康复患者实施整体性治疗,本文将运动链理论对FAI的临床康复实践进行总结,旨在为FAI的康复治疗提供新思路、新方法,丰富康复治疗手段。Functional ankle instability(FAI)is a condition of subjective neuromuscular and perceptual instability in the joint after ankle injury.It is common in daily life and sports.In patients with FAI,surrounding ligaments,joint capsules,muscles,tendons and other soft tissues of the ankle are impaired at varying degrees,with damaged proprioceptors,and abnormal subjective performance and voluntary movement.FAI can lead to decreased muscle recruitment of the ipsilateral lower limbs,prolonged reaction time,and decreased dynamic stability of the ankle joint.Moreover,it causes neuromuscular control disorders,such as ankle proprioception,peripheral muscle strength,and balance function.The treatment of FAI focuses on rehabilitation training to restoreankle function.At present,the movement chain theory has been gradually applied to the musculoskeletal system disease rehabilitation.Based on the integrity of movement chain theory,conductivity and biomechanical effect,the movement chain theory highlights a holistic treatment of musculoskeletal system rehabilitation.This paper summarized the movement chain theory applied to the rehabilitation of FAI,aiming to provide new ideas and methods to rich rehabilitation treatment.
