摘  要:目的探讨平板运动试验(treadmill exercise test,TET)在民航空勤人员日常体检鉴定中对鉴别心肌缺血、诊断冠心病、预防心血管突发意外事件引起空中失能的重要性。方法对中国民用航空飞行学院医院2019-01-01—2023-12-31接收的556例民航空勤人员TET结果进行分析,以及TET阳性后的进一步检查结果进行对比分析。结果556例民航空勤人员21例TET为阳性,23例为可疑阳性。进一步行冠状动脉CTA或冠状动脉造影检查,其中TET阳性中检出心肌桥15例,冠状动脉狭窄6例;可疑阳性中检出心肌桥10例,冠状动脉狭窄3例。结论经过研究分析,TET可广泛用于冠心病的诊断、鉴别诊断及其预后评价,对于可疑冠心病和评价冠心病的严重程度方面,在特殊职业中具有不可替代的作用。Objective To investigate the importance of treadmill exercise test(TET)in the routine physical examination and evaluation for identifying myocardial ischemia,diagnosing coronary heart disease(CHD)and preventing cardiovascular emergencies that may induce flight incapacitation in civil aviation flight crew members.Methods We analyzed TET results in 556 civil aviation flight crew members in Hospital of Civil Aviation Flight University of China from January 1,2019 to December 31,2023.Further examinations following positive TET results were analyzed.Results TET positivity was detected in 21/556 cases,and suspected TET positivity was detected in 23 cases.Among TET positive cases,further computed tomography angiography(CTA)or coronary angiography revealed myocardial bridges in 15 cases and coronary stenosis in 6 cases.Among the 23 cases with suspected TET positivity,myocardial bridges were detected in 10 cases,and coronary stenosis were detected in 3 cases.Conclusion TET can be widely used in the diagnosis,differential diagnosis,and prognosis evaluation of CHD.It plays an irreplaceable role in identifying suspected CHD and the assessment of the severity of CHD in special occupations.
