摘　　要：目的探究富血小板血浆联合闭合复位股骨近端防旋髓内钉(PFNA)内固定技术治疗高龄不稳定型股骨粗隆间骨折的价值。方法选择2021年5月至2022年8月上饶市立医院骨科收治的80例高龄不稳定型股骨粗隆间骨折患者作为研究对象,按照随机数字表法分为研究组(40例)和对照组(40例),对照组采用单纯PFNA内固定,研究组采用富血小板血浆联合PFNA进行治疗。比较两组的围手术期指标、髋关节功能、临床疗效、并发症发生情况以及生活质量。结果研究组术中出血量、下床活动时间、住院时间以及骨折愈合时间均低于对照组,差异有统计学意义(P<0.05)。整体分析结果显示,两组的Harris评分量表(HSS)评分时间、组间、交互作用比较,差异有统计学意义(P<0.05)。研究组治疗总有效率高于对照组,并发症总发生率低于对照组,差异有统计学意义(P<0.05)。两组术前简明健康调查量表(SF-36)评分比较,差异无统计学意义(P>0.05),术后研究组SF-36高于对照组,差异有统计学意义(P<0.05)。结论富血小板血浆联合PFNA内固定能够有效改善高龄不稳定型股骨粗隆间骨折患者的髋关节功能,促进患者恢复,提高治疗效果和生活质量,有利于降低患者并发症发生的可能性。Objective To explore the value of platelet-rich plasma combined with proximal femoral nail anti-rotation(PFNA)internal fixation in the treatment of elderly patients with unstable femoral intertrochanteric fractures.Methods Totally 80 elderly patients with unstable femoral intertrochanteric fractures admitted to Department of Orthopedics of Shangrao Municipal Hospital from May 2021 to August 2022 were selected as the research objects,and they were divided into study group(40 cases)and control group(40 cases)according to the random number table method.The control group received simple PFNA internal fixation,whereas the study group was treated with platelet-rich plasma combined with PFNA.Perioperative indicators,hip function,clinical efficacy,occurrence of complications and quality of life were compared between both groups.Results The intraoperative blood loss,ambulation time,hospital stay and fracture healing time in study group were lower than those in control group,the differences were statistically significant(P<0.05).Overall analysis showed that there was a statistically significant difference in Harris score scale(HSS)score from the aspects of time-point effect,between-group effect and interaction effect(P<0.05).The total effective rate of treatment in study group was higher compared with that in control group,the total incidence rate of complications was lower in study group compared to control group,and the differences were statistically significant(P<0.05).There was no statistical significance in 36-item short-form health survey scale(SF-36)score between the two groups before surgery(P>0.05).After surgery,the SF-36 score in study group was higher than that in control group,and the difference was statistically significant(P<0.05).Conclusion Platelet-rich plasma combined with PFNA internal fixation can effectively improve the hip function,promote the recovery,enhance the therapeutic effect and quality of life,and help to reduce the possibility of complications in elderly patients with unstable femoral intertr
