摘　　要：目的评估经皮穴位电刺激结合温针治疗乳腺癌相关上肢淋巴水肿的临床效果。方法选取2019年1月至2023年12月福建中医药大学附属康复医院收治的64例乳腺癌相关上肢淋巴水肿患者作为研究对象,按照随机数字表法分为对照组(32例)和治疗组(32例)。对照组采用手法淋巴引流、绷带加压包扎、护理和功能性锻炼的治疗方法,治疗组在对照组的基础上加用经皮穴位电刺激结合温针的治疗方法。比较两组患者治疗前后的腕关节横纹处、腕肘中点和肘腋横纹处肢体周长和肢体肿胀程度,比较两组患者的患肢肿胀程度以及不良事件发生率。结果治疗后两组患者各位置肢体周长短于本组治疗前,治疗组的各位置肢体周长短于对照组,差异有统计学意义(P<0.05);治疗组患肢肿胀程度低于对照组,差异有统计学意义(P<0.05)。两组患者均无严重不良事件发生。结论经皮穴位电刺激结合温针治疗可有效减轻乳腺癌相关上肢淋巴水肿患者的肢体周长和患肢肿胀程度,其临床效果优于仅使用传统的治疗方案,临床上可推广使用。Objective To evaluate the clinical effect of percutaneous acupoint electrical stimulation combined with warm acupuncture in the treatment of upper limb lymphedema associated with breast cancer.Methods A total of 64 patients with upper limb lymphedema associated with breast cancer admitted to the Rehabilitation Hospital Affiliated to Fujian University of Chinese Medicine from January 2019 to December 2023 were selected as the study objects,and they were divided into control group(32 cases)and treatment group(32 cases)according to random number table method.The control group was treated with manual lymphatic drainage,pressure bandaging,nursing and functional exercise.In the treatment group,percutaneous acupoint electrical stimulation combined with warm acupuncture was added on the basis of the control group.The limb circumference and limb swelling degree of the wrist joint,the midpoint of the wrist elbow and the transverse line of the elbow were compared between the two groups before and after treatment,and the swelling degree of the affected limb and the incidence of adverse events were compared between the two groups.Results After treatment,the limb length at each position of the two groups were higher than those before treatment,and the limb length at each position of the treatment group was higher than that of the control group,the differences were statistically significant(P<0.05).The swelling degree of the affected limb in the treatment group was lower than that in the control group,the difference was statistically significant(P<0.05).No serious adverse events occurred in either group.Conclusion Percutaneous acupoint electrical stimulation combined with warm acupuncture therapy can effectively reduce limb circumference and swelling degree of affected limb in patients with upper limb lymphedema associated with breast cancer,and its clinical effect is better than that of traditional treatment only,so it can be widely used in clinical practice.
