摘　　要：目的探讨血清肾损伤分子1(KIM-1)、胱抑素-C(Cys-C)、尿微量清蛋白(UmAlb)检测对早期糖尿病肾病(DN)的诊断价值。方法选取2019年5月至2022年4月九江市中医医院收治的114例糖尿病患者作为研究对象,评估患者早期DN发生情况并分为早期DN发生组、未发生组;检测并比较两组患者血清KIM-1、Cys-C、UmAlb水平,分析血清KIM-1、Cys-C、UmAlb联合检测对早期DN的诊断价值。结果114例糖尿病患者中有37例发生早期DN,发生率为32.46%;早期DN发生组血清KIM-1、Cys-C、UmAlb水平均高于未发生组,差异有统计学意义(P<0.05);绘制ROC曲线显示,血清KIM-1、Cys-C、UmAlb单独及联合检测诊断早期DN的AUC>0.080,均有一定诊断价值,且联合检测诊断价值最高。结论联合检测血清KIM-1、Cys-C、UmAlb诊断早期DN有较高应用价值,对临床诊治有重要指导意义。Objective To investigate the diagnostic value of serum kidney injury molecule 1(KIM-1),cystatin C(Cys-C)and urinary microalbumin(UmAlb)in early diabetic nephropathy(DN).Methods A total of 114 diabetic patients admitted to Jiujiang Hospital of Traditional Chinese Medicine from May 2019 to April 2022 were selected as the study objects to evaluate the occurrence of early DN and divided into early DN occurrence group and non-occurrence group.The levels of serum KIM-1,Cys-C and UmAlb were detected and compared between the two groups,and the value of combined detection of serum KIM-1,Cys-C and UmAlb in the diagnosis of early DN were analyzed.Results Early DN occurred in 37 of 114 diabetic patients(32.46%).The levels of KIM-1,Cys-C and UmAlb in the early DN group were higher than those of the non-DN group,and the differences were statistically significant(P<0.05).The ROC curve showed that the AUC value of serum KIM-1,Cys-C and UmAlb in the diagnosis of early DN were more than 0.080,which had certain diagnostic value,and the combined detection had the highest diagnostic value.Conclusion The combined detection of serum KIM-1,Cys-C and UmAlb in the diagnosis of early DN has high application value,and has important guiding significance for clinical diagnosis and treatment.
