改良Devine术联合阴茎阴囊角成形治疗隐匿性阴茎的临床价值分析

摘　　要：目的探究改良Devine术联合阴茎阴囊角成形治疗儿童隐匿性阴茎的临床应用价值。方法回顾性选取2021年1月—2024年1月江苏省启东市妇幼保健院外科收治的64例隐匿性阴茎患儿的临床资料,按照治疗方式的不同分为两组,对照组(32例)接受改良Devine术治疗,观察组(32例)在对照组基础上联合应用阴茎阴囊角成形治疗。比较两组患儿阴茎长度、术后延长长度、术后包皮水肿时间、住院时间、术后并发症发生情况。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。两组患者术后包皮水肿时间和住院时间比较,差异无统计学意义(P均>0.05)。观察组患儿术后3个月阴茎长度和术后延长长度均优于对照组,差异有统计学意义(P均<0.05)。观察组并发症发生率为18.75%(6/32),与对照组的25.00%(8/32)相比,差异无统计学意义(χ^(2)=0.171,P=0.680)。结论隐匿性阴茎患儿采用改良Devine术联合阴茎阴囊角成形治疗,可显著提高治疗成效,增加阴茎长度,对提高术后外观满意度和整体效果具有优势,治疗方案安全有效。Objective To explore the clinical application value of modified Devine technique combined with penile and scrotal angle shaping treatment in patients with mild occult penis.Methods The clinical data of 64 children with occulted penis admitted to the Department of Surgery at Qidong Maternal and Child Health Hospital in Jiangsu Province from January 2021 to January 2024 were retrospectively selected and divided into two groups according to different treatment methods.The control group(32 cases)received modified Devine technique,and the observation group(32 cases)received penile and scrotal angle shaping treatment on the basis of the control group surgery.The length of penis,the length of postoperative extension,the duration of postoperative prepuce edema,the length of hospital stay and the incidence of postoperative complications were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).There was no significant difference in the duration of postoperative prepuce edema and hospital stay between the two groups(both P>0.05).The penile length and postoperative extension length in the observation group were better than those in the control group at 3 months after operation,and the differences were statistically significant(both P<0.05).The incidence of complications in the observation group was 18.75%(6/32),compared with 25.00%(8/32)in the control group,the difference was not statistically significant(χ^(2)=0.171,P=0.680).Conclusion Modified Devine combined with penile and scrotal angle shaping can significantly improve the efficacy of treatment,increase the length of the penis,and improve the postoperative appearance satisfaction and overall effect.The treatment plan is safe and effective.
