立体定向微创联合尿激酶治疗高血压脑出血的临床效果分析

摘　　要：目的研究应用立体定向微创技术结合尿激酶治疗高血压脑出血的疗效。方法随机选取2020年6月—2023年6月兴义市人民医院收治的150例高血压脑出血患者为研究对象。根据不同治疗方法分为联合组和常规组,各75例。联合组采用立体定向微创结合尿激酶疗法,常规组予以开颅脑内血肿清除术。比较两组神经功能得分、临床指标、并发症发生情况。结果治疗后,联合组神经功能得分低于常规组,差异有统计学意义(P<0.05)。联合组的血肿消失时间、血肿旁水肿面积均优于对照组,首次清除血肿量多于对照组,差异有统计学意义(P均<0.05)。联合组术后并发症总发生率为1.33%(1/75),低于常规组的9.33%(7/75),差异有统计学意义(χ^(2)=4.754,P=0.029)。结论应用立体定向微创技术结合尿激酶治疗高血压脑出血患者疗效显著,可改善患者的神经功能,减轻血肿严重程度,且术后并发症较少,安全性较高。Objective To study the effect of minimally invasive stereotactic technique combined with urokinase in the treatment of hypertensive cerebral hemorrhage.Methods 150 patients with hypertensive cerebral hemorrhage treated in Xingyi People's Hospital from June 2020 to June 2023 were randomly selected as the study objects.They were divided into combined group and conventional group according to different treatment methods,with 75 cases in each group.The combined group was treated with minimally invasive stereotactic therapy combined with urokinase,and the conventional group was treated with intracerebral hematoma removal.The neurological function scores,clinical indicators and complications were compared between the two groups.Results After treatment,the neurological function score of combined group was lower than that of conventional group,and the difference was statistically significant(P<0.05).The time of hematoma disappearance and the area of edema near hematoma in the combined group were better than those in the control group,and the amount of initial hematoma removal was higher than that in the control group,the differences were statistically significant(all P<0.05).The total incidence of postoperative complications was only 1.33%(1/75)in the combined group,which was lower than 9.33%(7/75)in the conventional group,and the difference was statistically significant(χ^(2)=4.754,P=0.029).Conclusion The use of minimally invasive stereotactic technique combined with urokinase in the treatment of hypertensive cerebral hemorrhage patients is effective,can improve the neurological function of patients,reduce the severity of hematoma,and less postoperative complications,high safety.
