摘  要:探讨评估体素内不相干运动(IVIM)在鉴别诊断肾乏脂性血管平滑肌脂肪瘤(MFAML)与均质性肾透明细胞癌(CCRCC)中的应用价值。方法:对经术后病理证实的36例患者的影像资料进行回顾性分析,包括均质性肾透明细胞癌(24例)和肾乏脂性血管平滑肌脂肪瘤(12例)。所有患者术前均接受常规MRI及IVIM扫描。通过专业软件分析IVIM图像,获取相关参数值,并采用独立样本t检验和受试者工作特征曲线(ROC)评估其对两组肿瘤的鉴别能力。结果:均质性CCRCC与MFAML在IVIM参数灌注分数(f)和单纯扩散系数(D)上差异有统计学意义。结论:IVIM技术参数D与f有助于在临床实践中区分MFAML与均质性CCRCC,为这两种肿瘤的鉴别诊断提供了一种新的有效手段。Objective:To evaluate the value of intravoxel incoherent motion(IVIM)in the differential diagnosis of renal minimal-fat angiomyolipoma(MFAML)and homogeneous renal clear cell carcinoma(CCRCC).Methods:The imaging data of 36 patients confirmed by postoperative pathology were retrospectively analyzed,including homogeneous renal clear cell carcinoma(n=24)and renal minimal-fat angiomyolipoma(n=12).All patients underwent conventional MRI and IVIM scans before surgery.The IVIM images were analyzed by professional software to obtain the relevant parameter values,and the independent sample t test and ROC curve were used to evaluate the ability to identify the two groups of tumors.Results:There were significant differences in IVIM parameters perfusion fraction(F)and simple diffusion coefficient(D)between homogeneous renal clear cell carcinoma and renal lipid-poor angiomyolipoma.Conclusion:The parameters D and f of IVIM technology are helpful to distinguish MFAML from homogeneous CCRCC in clinical practice,which provides a new and effective method for the differential diagnosis of these two tumors.
