摘  要:目的分析贵阳市不明原因肺炎多种病原体混合感染情况,为完善不明原因肺炎预防控制及临床诊疗提供依据。方法通过实时荧光聚合酶链反应(real-time fluorescence PCR)对2020—2022年不明原因肺炎病例的咽拭子和痰液标本进行检测,在常规监测的人感染高致病性禽流感病毒和SARS冠状病毒的基础上,同时加入非常规监测的易引起肺炎症状的多种呼吸道病毒和细菌进行检测。结果457份标本中,病毒与细菌混合感染66份,感染率为14.4%;病毒与病毒混合感染9份,感染率为2.0%;细菌与细菌混合感染27份,感染率为5.9%。不同病原体混合感染的检出率差异有统计学意义(χ^(2)=53.955,P<0.001)。3种病原体混合感染标本14份,占3.1%,呼吸道合胞病毒和流感嗜血杆菌混合感染最多,共40份,占8.8%,流感嗜血杆菌和肺炎链球菌混合感染24份,占5.3%。结论贵阳市不明原因肺炎病例中存在多种病原体混合感染情况,以病毒与细菌混合感染为主。Objective To analyze the mixed infection of multiple pathogens in unexplained pneumonia in Guiyang City,and to provide a basis for improving the prevention and control of unexplained pneumonia and clinical diagnosis and treatment.Methods Real-time fluorescence polymerase chain reaction(real-time fluorescence PCR)was employed to examine pharyngeal swabs and sputum specimens of cases with unidentified pneumonia from 2020 to 2022.In addition to the routine monitoring of highly pathogenic avian Influenza viruses and SARS Coronaviruses in human infections,unconventional surveillance was conducted for multiple respiratory viruses and bacteria susceptible to causing pneumonia symptoms.Results Of the 457 specimens,66 cases of mixed infection by viruses and bacteria,with an infection rate of 14.4%.There were 9 cases of viral and viral mixed infection with an infection rate of 2.0%and 27 cases of bacterial-bacterial mixed infection with an infection rate of 5.9%.The difference was statistically significant in the detection rates of mixed infection with different pathogens(χ^(2)=53.955,P<0.001).There were 14 specimens of mixed infection by the three pathogens for 3.1%,with the highest number of mixed infection by Respiratory syncytial virus and Haemophilus influenzae of total for 40 copies with 8.8%,and mixed infection by Haemophilus influenzae and Streptococcus pneumoniae of 24 cases for 5.3%.Conclusion The cases of unexplained pneumonia in Guiyang City have mixed infection of multiple pathogens with predominance of viral and bacterial mixed infection.
