子午流注择时耳穴埋豆对稳定创伤性骨髓炎术前患者的影响

摘　　要：目的研究子午流注择时耳穴埋豆对创伤性骨髓炎术前患者血压稳定效果及焦虑状态的影响。方法选取2020年4月至2023年4月濮阳市中医医院骨五科收治的126例创伤性骨髓炎术前血压升高且有不同程度焦虑患者作为研究对象,采用随机数字表法将患者分为对照组(63例)和观察组(63例)。对照组男40例,女23例;年龄32~71(54.27±7.25)岁。观察组男38例,女25例;年龄34~70(53.61±6.71)岁。对照组采用常规术前管理,观察组在对照组基础上,于术前5 d采用子午流注择时行耳穴埋豆。两组患者均持续干预5 d。比较两组患者干预前后的血压稳定效果、疼痛情况[视觉模拟评分法(VAS)]、焦虑状态[汉密尔顿焦虑量表(HAMA)]、心理弹性水平[Connor-Davidson心理弹性量表(CD-RISC)]。采用独立样本t检验、配对t检验和χ^(2)检验。结果干预后,观察组收缩压[(130.51±4.22)mmHg](1 mmHg=0.133 kPa)、舒张压[(83.10±2.89)mmHg]、心率[(89.55±4.25)次/min]均低于对照组[(133.12±3.85)mmHg、(86.32±3.11)mmHg、(93.48±3.95)次/min](均P<0.05);观察组VAS[(2.96±0.65)分]、HAMA评分[(10.21±2.53)分]均低于对照组[(4.12±0.78)分、(13.26±2.92)分](均P<0.05);观察组CD-RISC评分[(87.83±5.80)分]高于对照组[(81.23±5.39)分](P<0.05)。结论子午流注择时耳穴埋豆能有效缓解创伤性骨髓炎术前患者的疼痛和焦虑,提高患者的心理弹性水平,稳定血压,具有临床推广价值。Objective To study the effect of auricular-point bean pressing of midnight-noon and ebb-flow doctrine on the stability of blood pressure and anxiety state in patients with traumatic osteomyelitis before operation.Methods A total of 126 patients with pre-traumatic osteomyelitis with elevated blood pressure and anxiety of different degrees admitted to the Fifth Department of Orthopedics,Puyang Hospital of Traditional Chinese Medicine from April 2020 to April 2023 were selected as the study objects,and were divided into a control group(63 cases)and an observation group(63 cases)by the random number table method.There were 40 males and 23 females in the control group,aged 32-71(54.27±7.25)years.There were 38 males and 25 females in the observation group,aged 34-70(53.61±6.71)years.The control group was given routine preoperative management,and the observation group was given auricular-point bean pressing of midnight-noon and ebb-flow doctrine since 5 days before surgery.The intervention lasted for 5 days in both groups.The effects of blood pressure stabilization,pain status[Visual Analogue Scale(VAS)],anxiety status[Hamilton Anxiety Scale(HAMA)],and mental resilience[Connor-Davidson Resilience Scale(CD-RISC)]before and after intervention were compared between the two groups.Independent sample t test,paired t test,andχ^(2)test were used.Results After intervention,the systolic blood pressure[(130.51±4.22)mmHg](1 mmHg=0.133 kPa),diastolic blood pressure[(83.10±2.89)mmHg],and heart rate[(89.55±4.25)beats/min]in the observation group were lower than those in the control group[(133.12±3.85)mmHg,(86.32±3.11)mmHg,and(93.48±3.95)beats/min](all P<0.05).The scores of VAS[(2.96±0.65)points]and HAMA[(10.21±2.53)points]of the observation group were lower than those of the control group[(4.12±0.78)points and(13.26±2.92)points](both P<0.05).The CD-RISC score of the observation group was higher than that of the control group[(87.83±5.80)points vs.(81.23±5.39)points](P<0.05).Conclusion Auricular-point pressing bean of m
