摘 要:目的:观察母体孕前超重肥胖和婴幼儿超重肥胖对2岁内婴幼儿缺铁性贫血的影响。为社区妇幼保健及降低儿童缺铁性贫血发生率提供线索和依据。方法:选取2021年1月1日至2023年12月31日在上海市黄浦区外滩街道卫生服务中心体检的611对母子为研究对象,建立回顾性队列研究。提取孕前体质指数(pBMI)和子代2岁内的体质量、身高、血红蛋白及计算身长别体质量等数据,采用logistic回归对母体孕前及婴幼儿超重肥胖与婴幼儿缺铁性贫血的相关性进行分析。结果:以母亲pBMI作为自变量进行logistic回归分析,与正常pBMI相比,孕前超重肥胖与6月龄、1岁、2岁婴幼儿发生缺铁性贫血风险的AOR=2.426、2.314、4.369,95%CI(1.351,4.356)、(1.125,4.761)、(1.261,15.137);以婴幼儿超重肥胖为自变量进行logistic回归分析,在儿童1岁和2岁时超重肥胖会显著增加缺铁性贫血的风险,AOR=2.537、4.159,95%CI(1.186,5.429)、(1.205,14.348),P均<0.05。结论:母亲孕前超重肥胖和婴幼儿超重肥胖均是婴幼儿1岁和2岁缺铁性贫血发生的独立危险因素。Objective:To observe the effect of maternal overweight and obesity before pregnancy and infant overweight and obe-sity on Iron-deficiency anemia in infants under 2 years old.To provide clues and basis for community maternal and child health care and reduce the incidence of Iron-deficiency anemia in children.Methods:A retrospective Cohort study was conducted on 6ll pairs of mothers and children who underwent physical examination at the Waitan Street Health Service Center in Huangpu District of Shanghai from January 1,2021 to December 31,2023.The data of pre pregnancy body mass index(pBMI),weight,height,hemoglobin and calculated length weight of the offspring within 2 years old were extracted,and the correlation between pre pregnancy and infant overweight and obesity and infant Iron-deficiency anemia was analyzed by logistic regression.Results:In the logistic regression analysis with the mother's pBMI as an independent variable,compared with the normal pBMI,the AOR of pre pregnancy overweight and obesity on the risk of Iron-deficiency anemia in infants aged 6 months,1 year,and 2 years old was 2.426;2.314;4.369,95%CI(1.351,4.356);(1.125,4.761);(1.261,15.137);In the logistic regression analysis with infant overweight and obesity as independent variable,overweight and obesity of children at the age of 1 and 2 sig-nificantly increased the risk of Iron-deficiency anemia of children(AOR=2.537);4.159,95%CI(1.186,5.429);(1.205,14.348),P<0.05.Conclusion:Pre-pregnancy overweight and obesity of mothers and overweight and obesity of infants are independent risk factors for the occurrence of Iron-deficiency anemia in infants aged 1 and 2 years.
