冠心病中成药治疗的门诊处方分析及合理性评估

摘　　要：目的通过收集并探讨分析武汉市某医院用于治疗冠心病的中成药门诊处方,总结关于冠心病中医辨证施治及中成药处方的规律。方法调取2023年12月至2024年4月医院实施处方点评后开具的1000张处方,对其中治疗冠心病中成药的使用情况进行统计,并对冠心病患者中成药处方药物种类、使用频率、使用合理情况进行分析。结果在1000张中成药处方中,最常用的中成药有10种,活血化瘀和化瘀止血类药物使用频率居于前列,其中麝香保心丸(21.00%)、速效救心丸(10.90%)、复方丹参滴丸(10.20%)较为常见。中成药主要药物组成有34种,前三位药物为丹参(12.63%)、川芎(10.05%)及人参/红参(9.80%)。在1000张处方中,不合理处方有234张,占23.40%,其中不规范处方有139张(59.40%),主要存在问题:内容缺失有54张(23.07%),疗程不当有46张(19.65%),缺少中医辨证有39张(16.67%)。用药不适宜处方有87张(37.17%),主要存在问题:适应证不适宜有47张(20.08%),用量、用法不适宜有40张(17.09%)。其他不适宜处方有8张(3.42%)。结论治疗冠心病的中成药种类较多,在用药过程中,需注意中医辨证及用药监护,进一步加强合理用药,提高用药水平。Objective:To collect and analyze the outpatient prescriptions of Chinese patent medicine for treating coronary heart disease in a hospital in Wuhan,and summarize the rules of TCM syndrome differentiation and treatment of coronary heart disease and Chinese patent medicine prescriptions.Methods:1000 prescriptions issued by the hospital from December 2023 to April 2024 were collected,and the use of proprietary Chinese medicines in patients with coronary heart disease was counted,and the types、frequency and rational use of proprietary Chinese medicines in patients with coronary heart disease were analyzed.Results:Among 1000 prescriptions of Chinese patent medicines,there are 10 kinds of Chinese patent medicines that are most commonly used,among which drugs for promoting blood circulation and removing blood stasis and stopping bleeding are in the forefront,among which Musk Baoxin Pill(21.00%),Suxiao Jiuxin Pill(10.90%)and Compound Danshen Dropping Pill(10.20%)are more common.There are 34 kinds of main Chinese patent medicines,and the top three medicines are Danshen(12.63%),Chuanxiong(10.05%)and Ginseng/Ginseng Rubra(9.80%).Among the 1000 prescriptions,234 were irrational prescriptions,accounting for 23.40%,of which 139 were nonstandard prescriptions(59.40%).The main problems were:54 were lack of content(23.07%),46 were improper treatment(19.65%)and 39 were lack of TCM syndrome differentiation(16.67%).There are 87 prescriptions(37.17%)that are not suitable for medication,and the main problems are:47 prescriptions are not suitable for indications(20.08%),and 40 prescriptions are not suitable for dosage and usage(17.09%).There were 8 other inappropriate prescriptions(3.42%).Conclusion:There are many kinds of Chinese patent medicines for treating coronary heart disease.In the process of medication,we should pay attention to TCM syndrome differentiation and medication monitoring,further strengthen rational medication and improve the level of medication.
