摘　　要：顽固性高血压(RH)是一种严重的高血压,与心血管风险增加有关。虽然接受降压治疗受RH影响的患者数量较少,但绝对数量较高,而且还在继续增加。这些患者需要筛查继发性高血压和假耐药性,包括对处方药的依从性差和白大褂现象。RH的治疗包括生活方式的改变和药物治疗,其中生活方式的改变包括饮食调整、减重、体育锻炼和限制饮酒;药物治疗包括利尿剂、盐皮质激素受体拮抗剂、β受体阻滞剂、血管紧张素Ⅱ受体拮抗剂等。目前,介入治疗已成为辅助治疗的选择。Resistant hypertension(RH)is a severe form of hypertension associated with increased cardiovascular risk.Although the number of patients receiving antihypertensive treatment affected by RH is relatively small,the absolute number is high and continues to increase.These patients need to be screened for secondary hypertension and pseudoresistance,including poor adherence to prescribed medicines and the white-coat phenomenon.The treatment of RH consists of lifestyle modifications and medication.Among them,lifestyle modifications include dietary adjustments,weight loss,physical exercise,and limiting alcohol consumption;medication includes diuretics,mineralocorticoid receptor antagonists,βreceptor blockers,angiotensinⅡreceptor antagonist etc.At present,interventional therapy has become the choice for adjuvant therapy.
