组合式FURS治疗复杂性输尿管上段结石的效果

摘　　要：目的探讨组合式输尿管软镜钬激光碎石术(FURS)治疗复杂性输尿管上段结石的效果。方法将我院收治的复杂性输尿管上段结石患者145例,按照随机数字表法分为两组,对照组72例采用经皮肾镜碎石术(PCNL),观察组73例采用组合式FURS。观察两组手术情况、结石清除率、生活质量、并发症发生率。结果观察组手术时间及住院时间均短于对照组、术中出血量少于对照组(P<0.05);观察组一次性结石清除率高于对照组(P<0.05);随访结束后观察组SF-36生活质量量表评分均高于对照组(P<0.05);观察组腹部疼痛、术后感染、恶心呕吐、血尿、发热、尿道狭窄发生率均低于对照组,但组间对比无统计学意义(P>0.05)。结论将组合式FURS治疗应用于复杂性输尿管上段结石可缩短手术时间、减少术中出血量,术后并发症发生率更低,生活质量高。Objective To investigate the effect of combined ureteroflexible holmium laser lithotripsy(FURS)in the treatment of complex upper ureteral calculi.Methods 145 patients with complex upper ureteral calculi admitted to our hospital were selected and randomly divided into the control group and the observation group.72 cases in the control group underwent percutaneous nephrectomy(PCNL),and 73 cases in the observation group underwent combined FURS.The operation condition,stone clearance rate,quality of life and complication rate of the two groups were observed.Results The operation time and hospitalization time in the observation group were shorter than those in the control group,and the amount of intraoperative blood loss was less than that in the control group(P<0.05).The one-time stone clearance rate of observation group was higher than that of control group(P<0.05).After follow-up,the scores of SF-36 quality of life scale in observation group were higher than those in control group(P<0.05).The incidence of abdominal pain,postoperative infection,nausea and vomiting,hematuria,fever and urethral stenosis in observation group was lower than that in control group,but there was no significant difference between the two groups(P>0.05).Conclusion Combined FURS treatment for complex upper ureteral calculi can shorten operation time,reduce intraoperative blood loss,lower postoperative complications and improve life quality.
