子宫腺肌病行宫腔镜下LNG-IUS套马结式固定术的临床疗效

摘　　要：目的评估子宫腺肌病患者行宫腔镜下左炔诺孕酮宫内缓释系统(LNG-IUS)套马结式固定术的安全性和有效性。方法选取我院收治的子宫腺肌病患者60例,随机分为观察组和对照组,每组30例。观察组行宫腔镜下LNG-IUS套马结式固定术,对照组注射GnRH-a类药物治疗4~6个疗程后放置LNGIUS。对比分析两组患者治疗后LNG-IUS发生下移或脱落情况、痛经情况、血红蛋白值及不良反应情况。结果观察组患者术后发生下移或脱落1例,对照组患者术后出现下移或脱落14例,两组脱落或下移发生率比较,P<0.05。治疗6个月后两组痛经情况、血红蛋白值比较,观察组明显优于对照组(P<0.05)。观察组患者手术过程顺利,均未发生不良反应,对照组患者在注射GnRH-a类药物过程中有5人出现潮热、盗汗、睡眠差、浑身疼痛不适等更年期症状。结论子宫腺肌病患者行宫腔镜下LNG-IUS套马结式固定术安全有效,能明显降低其在子宫腺肌病大子宫内的脱落风险。Objective To evaluate the safety and efficacy of hysteroscopic levonorgestrel intrauterine slow-release system(LNG-IUS)hitch fixation in the treatment of uterine adenomyopathy.Methods Sixty uterine adenomyopathy patients were randomly divided into the control group and the observation group,30 cases in each group.The observation group underwent hysteroscopic LNG-IUS hitch fixation,and the control group received LNG-IUS after 4~6 courses of GnRH-a injection.The displacement or shedding of LNG-IUS,dysmenorrhea,hemoglobin value and adverse reactions were compared between the two groups.Results One patient in the observation group experienced downward displacement after surgery,14 patients in the control group experienced downward displacement or dislocation after surgery,and the comparison of dislocation or displacement rate between the two groups was with P<0.05.6 months after surgery,the dysmenorrhea,hemoglobin value in the observation group were better than those in the control group(P<0.05).There was no adverse reactions in the observation group,5 patients in the control group had hot flashes,night sweats,poor sleep,aches and pains and other symptoms of menopause.Conclusion LNG-IUS hitch fixation under hysteroscopy is safe and effective,and can significantly reduce the risk of detachment in uterine adenomyopathy.
