摘 要:目的探究吲哚布芬对使用带隧道和涤纶套导管(TCC)作为血管通路的维持性血液透析患者导管功能、血栓形成及凝血功能的效果。方法纳入本院98例使用TCC作为维持性血液透析(MHD)血管通路的患者作为研究对象,随机分为观察组和对照组,每组49例。对照组给予阿司匹林治疗,观察组给予吲哚布芬治疗。比较两组TCC血流量、静脉压、导管功能不良发生率、导管首次血栓形成时间、治疗前后血小板(PLT)计数、凝血功能以及抗PLT药物相关的不良反应。结果置管半年时,观察组导管血流量高于对照组、导管静脉压低于对照组,导管功能不良发生率低于对照组(P<0.05);观察组TCC血栓首次形成时间均较对照组长(P<0.05);置管半年时,两组PLT水平高于入组时(P<0.05),对照组APTT水平低于入组时(P<0.05),两组PLT水平及凝血功能各项指标水平比较,P>0.05;两组不良反应发生率比较,P>0.05。结论吲哚布芬能有效改善使用TCC作为血管通路的维持性血液透析患者导管功能,降低导管功能不良反应发生率,预防血栓形成,且不增加出血风险,药物相关不良反应较少,安全性较高。Objective To investigate the effect of indobufen on catheter function,thrombosis and coagulation of maintenance hemodialysis patients using tunnel and dacron catheter(TCC)as vascular access.Methods 98 patients using TCC as vascular access for maintenance hemodialysis(MHD)were included in the study and randomly divided into the control group and the observation group,49 cases in each group.The control group were given aspirin,and the observation group were given indobufen.The blood flow and venous pressure of TCC,the incidence of catheter malfunction,the time of first catheter thrombosis were compared between the two groups.Platelet(PLT)count,coagulation function and adverse reactions associated with anti-PLT drugs before and after treatment were compared.Results After six months of catheterization,catheter blood flow in the observation group was higher than the control group,catheter venous pressure was lower than control group,and the incidence of catheter malfunction was lower than control group(P<0.05).The first formation time of TCC thrombus in observation group was longer than that in control group(P<0.05).After six months of catheterization,PLT levels in the two groups were higher than those before treatment(P<0.05),APTT level in the control group was lower than that before treatment(P<0.05),and there were no statistically significant difference in PLT level and coagulation function indexes between the two groups(P>0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Indobufen can effectively improve catheter function,reduce the incidence of adverse reactions to catheter function and prevent thrombosis in maintenance hemodialysis patients using TCC as vascular access,which has lower risk of bleeding,fewer drug-related adverse reactions and high safety.
