家庭医生签约服务模式对社区老年高血压患者规范化管理的效果观察

摘　　要：目的观察采取家庭医生签约服务模式对社区老年原发性高血压病患者进行健康管理的效果。方法选择2022年1月—2023年1月200例社区高血压患者为对象,依据社区登记顺序分为对照组和观察组,每组100例。对照组采取常规指导,观察组采取家庭医生签约团队服务。对比两组的满意度、血压指标和生活方式改善程度。结果两组血压水平存在明显差异(P<0.05);观察组生活方式改善率差异明显优于对照组(P<0.05);观察组满意度优于对照组,差异具有统计学意义(P<0.05)。结论为社区高血压患者实施家庭医生签约服务团队配合干预,可改善患者不良的生活方式,稳定血压水平,病情管控效果理想。Objective To observe the effect of adopting a family doctor contract service model on health management of elderly patients with primary hypertension in the community.Methods 200 community hypertensive patients from January 2022 to January 2023 were selected as the subjects,and were divided into a control group and an observation group based on the order of community registration,with 100 cases in each group.The control group received routine guidance,while the observation group received family doctor contracted team services.Compare the satisfaction,blood pressure indicators,and improvement in lifestyle between the two groups.Results There was a significant difference in blood pressure levels between the two groups(P<0.05);The difference in improvement rate of lifestyle in the observation group was significantly better than that in the control group(P<0.05);The satisfaction of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Implementing family doctor contracted service team cooperation intervention for community hypertension patients can improve their unhealthy lifestyle,stabilize blood pressure levels,and achieve ideal disease control effects.
