摘　　要：目的分析比较顺行单根弹性髓内钉内固定术与微型钢板内固定术治疗第五掌骨颈骨折短期临床疗效.方法我院自2018年10月至2021年10月收治移位明显、不稳定的AO/OTA分型2C1-2C2型第五掌骨颈骨折患者52例,采用顺行单根弹性髓内钉内固定术(治疗组)27例,采用切开复位微型钢板内固定术(对照组)25例,对比两组患者的手术时间、术中C臂机透视次数、骨折愈合时间、DASH上肢功能评定等指标,评价两种手术方式的疗效.结果两组的手术时间、术中C臂机透视次数、骨折愈合时间、术后1个月和3个月DASH评分的差异均有统计学意义(P<0.05).结论第五掌骨颈骨折采用顺行单根弹性髓内钉内固定术治疗具有微创、操作简单、手术时间短、骨折愈合时间短、功能恢复好等优点,临床疗效满意,适宜临床推广.Objective To analyze and compare the short-term clinical efficacy of antegrade single elastic intramedullary nail and miniplate internal fixation in the treatment of fifth metacarpal neck fractures.Methods From October 2018 to October 2021,52 patients with AO/OTA classification 2C1 to 2C2 fifth metacarpal neck fractures with significant displacement and instability were treated.A total of 27 patients underwent antegrade single elastic intramedullary nail internal fixation surgery(treatment group)and 25 patients underwent open reduction miniplate internal fixation surgery(control group).The surgical time,intraoperative C-arm fluoroscopy frequency,fracture healing time,and DASH upper limb function evaluation were compared between the two groups to evaluate the efficacy of the two surgical methods.Results The differences in surgical time,intraoperative C-arm fluoroscopy frequency,fracture healing time,and DASH scores at 1 and 3 months after surgery between the two groups were statistically significant(P<0.05).Conclusion The use of antegrade single elastic intramedullary nail internal fixation for the treatment of fifth metacarpal neck fractures has the advantages of minimally invasive,simple operation,short surgical time,short fracture healing time,and good functional recovery.The clinical efficacy is satisfactory and suitable for clinical promotion.
