养阴润肠方联合针刺治疗老年慢传输型便秘临床观察

摘　　要：目的:观察养阴润肠方联合针刺治疗老年慢传输型便秘临床疗效。方法:选取老年慢传输型便秘患者84例,根据随机数字表法分为两组各42例。对照组予针刺治疗,治疗组在对照组基础上联合养阴润肠方治疗,疗程均为4周。观察两组患者临床疗效、中医证候评分、血清胃肠激素水平、结肠动力变化、肠道菌群数量检测及便秘患者生活质量量表(PAC-QOL)评分。结果:治疗组总有效率92.86%高于对照组的76.19%(P﹤0.05)。治疗后两组中医证候评分、血清VIP、5-HT水平、右半结肠通过时间、左半结肠通过时间、全结肠通过时间、大肠埃希菌平均数量、PAC-QOL评分均较治疗前明显降低(P﹤0.05),且治疗组各指标水平均低于对照组(P﹤0.05);治疗后两组乳酸杆菌数量、双歧杆菌数量、血清SP水平较治疗前上升(P﹤0.05),且治疗组均高于对照组(P﹤0.05)。结论:养阴润肠方联合针刺治疗老年慢传输型便秘可提高肠道动力,改善肠道菌群和胃肠激素水平,有效缓解临床症状,提高患者生活质量。Objective:To observe the clinical efficacy of Yangyin Runchang formula and acupuncture on elderly slow transit constipation.Methods:84 cases of elderly patients with slow transit constipation were selected and randomly divided into two groups,42 cases in each group.The control group was treated with acupuncture,while the treatment group was treated plus with Yangyin Runchang formula on the basis of control group.For both groups,the course of treatment was 4 weeks.Clinical efficacy,TCM syndrome score,level of serum gastrointestinal hormone,change of colonic motility,number of detected intestinal flora and score of Patient-Assessment of Constipation Quality Of Life(PAC-QOL)of patients were observed in two groups before and after treatment.Results:The total effective rate of treatment group was 92.86%,which was higher than 76.19%of control group(P﹤0.05).In both groups after treatment,TCM syndrome score,levels of serum VIP and 5-HT,right-sided colon transit time,left-sided colon transit time,total colon transit time,average number of Escherichia coli and PAC-QOL score were significantly decreased compared with before treatment(P﹤0.05),and the level of each index of treatment group was lower than that of control group(P﹤0.05).In both groups after treatment,number of lactobacilli,number of bifidobacterium and level of serum SP were increased compared with before treatment(P﹤0.05),and the treatment group was higher than control group(P﹤0.05).Conclusion:Yangyin Runchang formula and acupuncture for treating elderly slow transit constipation can enhance the intestinal motility,improve the levels of intestinal flora and gastrointestinal hormone,effectively relieve the clinical symptoms and improve patients'quality of life.
