手足外科住院患者细菌感染耐药表型及药敏分析

摘　　要：目的了解手足外科住院患者细菌感染种类及耐药性,为临床合理用药提供理论支持。方法2020年1月—2022年6月某院手足外科送检的伤口及脓液分泌物经接种平板培养后,使用全自动细菌鉴定药敏仪确定细菌种类及药敏结果,纸片法验证。结果共检出细菌358株,185株(51.68%)革兰阴性杆菌中肠杆菌目细菌115株(62.16%);172株(48.04%)革兰阳性球菌中金黄色葡萄球菌140株(81.40%),耐甲氧西林金黄色葡萄球菌27株(19.29%);革兰阳性杆菌1株(0.28%)。肠杆菌目细菌对青霉素类和一代头孢类药物耐药率达80%,金黄色葡萄球菌对青霉素类抗菌药物哌拉西林耐药率达96.83%。结论手足外科患者感染细菌种类多,耐药率高,应合理使用抗菌药物,避免耐药菌株的产生和传播。Objective To understand the types and drug resistance of bacterial infection in hospitalized patients of hand and foot surgery department,provide theoretical support for rational drug use in clinical practice.Methods The wounds and purulent secretions were inoculated and cultured on agar plates from hand and foot surgery department of a hospital from Jannary 2020 to June 2022,and verified by the kirby-Bcuer method.Results A total of 358 bacterial strains were detected.185 strains(51.39%)of Gram negative bacteria,including 115 strains(62.16%)of Enterobacteriaceae,172 strains(48.04%)of Gram positive cocci,including 140 strains(81.40%)of Staphylococcus aureus,27 strains(19.29%)of methicillin-resistant Staphylococcus aureus,1 Gram positive bacterial strain(0.28%).The resistance rate reached about 80%to Penicillin and first generation Cephalosporins of Enterobacteriaceae.The resistance rate reached 96.83%to Piperacillin of Penicillin antibiotics of Staphylococcus aureus.Conclusion There were many types of bacterial infections and high resistance rates in patients of hand and foot surgery department,antibiotics should be used reasonably to avoid the generation and spread of drug-resistant strains.
