针刺联合针刀松解华佗夹脊穴治疗慢性萎缩性胃炎胃络瘀血证40例临床观察

摘　　要：目的观察针刺联合针刀松解华佗夹脊穴治疗慢性萎缩性胃炎胃络瘀血证的临床疗效。方法将80例慢性萎缩性胃炎胃络瘀血证患者采用随机数字表法分为对照组和治疗组,各40例。对照组予以常规针刺,主穴取足三里(双侧)、内关(双侧)、公孙(双侧)、中脘,配穴取血海(双侧)、章门(双侧),得气后行捻转泻法,每周治疗5次;治疗组在对照组治疗方法的基础上联合针刀松解T5—T8双侧华佗夹脊穴,每周治疗2次。2组均连续治疗4周。比较2组的临床疗效及治疗前后胃黏膜病理组织评分。结果对照组总有效率为77.5%(31/40),治疗组为95.5%(38/40),2组比较差异有统计学意义(P<0.05);治疗后2组胃黏膜病理组织评分均明显降低,与同组治疗前比较差异有统计学意义(P<0.01),且治疗组降低更明显,与对照组治疗后比较差异有统计学意义(P<0.01)。结论针刺联合针刀松解华佗夹脊穴治疗慢性萎缩性胃炎胃络瘀血证临床疗效显著,可修复胃黏膜,改善胃功能,提高患者生活质量,值得临床推广应用。Objective To observe the clinical efficacy of acupuncture in combination with acupotomy at Huatuo Jiaji(Ex-B2)in the treatment of chronic atrophic gastritis with stomach collateral blood stasis syndrome.Methods 80 patients with chronic atrophic gastritis and stomach collateral blood stasis syndrome were divided into a control group and a treatment group using the random number table method,with 40 cases in each group.The control group received conventional acupuncture treatment.The main acupuncture points were Zusanli(ST36)(bilateral),Neiguan(PC6)(bilateral),Gongsun(SP4)(bilateral),and Zhongwan(RN12).The combined acupuncture points were Xuehai(SP10)(bilateral)and Zhangmen(LR13)(bilateral).The twisting and reducing manipulations were performed after obtaining qi.The treatment was administered five times a week.In the treatment group,on the basis of the treatment given to the control group,acupotomy was performed at bilateral Huatuo Jiaji at T5-T8,twice a week.Both groups were treated for four weeks.The clinical efficacy and pathological tissue scores of the gastric mucosa before and after treatment were compared between the two groups.Results The total effective rate was 77.5%(31/40)in the control group and 95.5%(38/40)in the treatment group.The difference was statistically significant(P<0.05).After treatment,the pathological tissue scores of the gastric mucosa in both groups decreased significantly.The difference was statistically significant when compared to that before treatment(P<0.01),and the decrease was more pronounced in the treatment group.The difference was statistically significant compared to the control group after treatment(P<0.01).Conclusion The combination of acupuncture and acupotomy at Huatuo Jiaji in the treatment of chronic atrophic gastritis with stomach collateral blood stasis shows a remarkable clinical effect.It can restore the gastric mucosa,improve gastric function,and enhance the patients'quality of life,and is worthy of clinical application.
