摘 要:目的探究蒙药白脉散有效成分组(BMECG)与曲安奈德局部应用对坐骨神经损伤大鼠的神经再生作用。方法采用随机对照研究。选用2月龄,体重240~260g,SD大鼠30只,按照随机数字表法将其随机分为5组:模型组(Model组)、空白组(Sham组)、白脉散有效成分组(BMECG组)、曲安奈德组(TA组)以及BMECG联合曲安奈德组(BMECG+TA组),每组6只。Model组仅建立坐骨神经损伤模型Sham组仅暴露坐骨神经后缝合;BMECG组建立模型后,每日灌胃BMECG150mg/kg,1次/d,持续4周;TA组建立模型后,在坐骨神经手术处应用1ml醋酸曲安奈德注射液;BMECG+TA组综合以上两组操作。测定所有大鼠术前1d、术后1d及术后1、2、3、4周的热刺激缩足反应潜伏期(PWTL)。取腰膨大段做免疫组化对iNOS分析,其中目标蛋白为棕黄色,对结果图像进行积分光密度分析;取坐骨神经做免疫荧光染色对神经元骨架蛋白NF200分析,其中目标蛋白呈绿色,并进行平均荧光强度分析。采用SPSS26.0软件进行统计学分析。结果PWTL值组别主效应(F=96.24,P<0.001),时间主效应(F=85.76,P<0.001)、组别和时间的交互效应(F=12.44,P<0.001)均显著。进一步分析,术后1、2、3、4周,Model组大鼠PWTL值均低于用药组(P均<0.05)。术后4周,BMECG+TA组PWTL值均高于BMECG组和TA组(P均<0.05)。与Model组相比,用药组iNOS平均光密度均较低(P均<0.05);与TA组和BMECG组相比,BMECG+TA组iNOS平均光密度较低(P均<0.05)。与Model组相比,用药组NF200平均荧光强度均较高(P均<0.05);与TA组和BMECG组相比,BMECG+TA组NF200平均荧光强度较高(P均<0.05)。结论白脉散有效成分组联合曲安奈德通过改善损伤处内环境、促进轴突生长,对坐骨神经有再生作用,且优于单一用药。Objective To explore the neuroregenerative effects of Mongolian medicine baimai effective compounds group(BMECG)and triamcinolone acetonide on the sciatic nerve injury in rat model.Methods A randomized controlled study was conducted.Thirty SD rats,aged 2 months,weighing 240-260 g,were randomly divided into 5 groups by the random number table:Model group,Sham group,BMECG group,triamcinolone acetonide group(TA group),and BMECG combined with triamcinolone acetonide group(BMECG+TA group),6 rats in each group.A model of sciatic nerve injury was established in the Model group,and only the sciatic nerve was exposed in the Sham group.After establishing the model,BMECG(150 mg/kg,once a day)was orally administered daily for four weeks in the BMECG group;triamcinolone acetonide(1 ml)was injected near the site of sciatic nerve exposure in the TA group;BMECG combined with triamcinolone acetonide in the BMECG+TA group.Paw withdraw thermal latency(PWTL)was measured at 1 day before the operation and 1 day,1 week,2 weeks,3 weeks,and 4 weeks after the operation.The lumbar enlargement segment was taken for immunohistochemical analysis of iNOS,the target protein was brownish yellow color,and the result image was analyzed by integral optical density analysis.The sciatic nerve was taken for immunofluorescence staining to analyze the neuronal cytoskeletal protein NF200,the target protein was green color,and the average fluorescence intensity of the result image was analyzed.SPSS 26.0 software was used for statistic analysis.Results The group main effect of PWTL(F=96.24,P<0.001),time main effect(F=85.76,P<0.001)and interaction effect among all groups(F=12.44,P<0.001)were all significant.At 1,2,3,and 4 weeks after the operation,PWTL was significantly higher in all the medication groups than that in the Model group(all P<0.05).At 4 weeks after the operation,PWTL was higher in the BMECG+TA group than that of the BMECG group and TA group(all P<0.05).Compared with the Model group,the average optical density of iNOS was significantly lower in
