摘  要:目的通过有限元模型分析正常轴向载荷下人体不同姿势对腰椎间盘的影响。方法采用多排螺旋CT和MRI对24名健康成年男性腰椎进行扫描,将采集到的腰椎图像数据导入Mimics Research 20.0三维重建软件进行L_(4)、L_(5)椎体及椎间盘模型重建,模型导入有限元分析软件,模拟正常站立、前屈、后伸、扭转、侧弯5种人体不同姿势对L_(4、5)椎间盘应力及应变的影响。结果正常站立时L_(4、5)椎间盘的应力为(85.72±5.74)N,椎间盘应变为0.86±0.08;人体前屈时L_(4、5)椎间盘的应力为(89.53±7.64)N,椎间盘应变为0.85±0.06;人体后伸时L_(4、5)椎间盘的应力为(90.36±6.07)N,椎间盘应变为0.83±0.07;人体侧弯时L_(4、5)椎间盘的应力为(159.06±8.42)N,椎间盘应变为0.59±0.12;人体水平扭转时L_(4、5)椎间盘的应力为(155.76±6.58)N,椎间盘应变为0.62±0.11。人体不同姿势下L_(4、5)椎间盘的应力差异有统计学意义(F=706.260,P<0.001),与人体正常站立、人体前屈、人体后伸时相比,人体扭转、侧弯时L_(4、5)椎间盘的应力更大,差异有统计学意义(P<0.001)。人体不同姿势下L_(4、5)椎间盘的应变差异有统计学意义(F=65.530,P<0.001),与人体正常站立、人体前屈、人体后伸时相比,人体扭转、侧弯时L_(4、5)椎间盘的应变更小,差异有统计学意义(P<0.001)。结论L_(4、5)椎间盘抵抗轴向压缩和前后弯曲的能力大于左右侧弯和水平扭转,人体左右侧弯和水平扭转状态下叠加的载荷更容易引起腰椎间盘损伤。Objective To analyze the influence of different postures of human body on lumbar intervertebral disc under normal axial load by finite element model.Methods The lumbar spine of 24 healthy adult males was scanned by multi-slice spiral CT and MRI.The lumbar spine image data were imported into Mimics Research 20.0 three-dimensional reconstruction software to reconstruct L_(4)and L_(5)vertebral body and intervertebral disc models,and the models were imported into finite element analysis software.The stress and strain of L_(4,5)intervertebral disc were simulated in five different human postures,including normal standing,flexion,extension,torsion and lateral bending.Results In normal standing,the stress of L_(4,5)intervertebral disc was(85.72±5.74)N,and the strain of intervertebral disc was 0.86±0.08.The stress of L_(4,5)intervertebral disc was(89.53±7.64)N,and the strain of intervertebral disc was 0.85±0.06.In extension,the stress of L_(4,5)intervertebral disc was(90.36±6.07)N,and the strain of intervertebral disc was 0.83±0.07.During lateral bending,the stress of L_(4,5)intervertebral disc was(159.06±8.42)N,and the strain of L_(4,5)intervertebral disc was 0.59±0.12.The stress of L_(4,5)intervertebral disc was(155.76±6.58)N,and the strain of intervertebral disc was 0.62±0.11.There were significant differences in the stress of L_(4,5)intervertebral discs in different postures(F=706.260,P<0.001).Compared with the normal standing,flexion and extension,the stress of L_(4,5)intervertebral discs in torsion and lateral bending was greater,and the difference was statistically significant(P<0.001).There were significant differences in the strain of L_(4,5)intervertebral discs in different human postures(F=65.530,P<0.001).Compared with the normal standing,flexion and extension,the strain of L_(4,5)intervertebral discs in torsion and lateral bending were smaller,and the difference was statistically significant(P<0.001).Conclusion The resistance of L_(4,5)intervertebral disc to axial compression and anterior-posterior b
