郭志华教授从“血不利则为水”论治心力衰竭水肿经验

摘　　要：郭志华教授基于“血不利则为水”理论,结合心、肺、脾的生理和病理特点,将“治病必求于本”与“五脏一体观”的辨证论治思想贯穿于心力衰竭合并顽固性水肿临床治疗过程。郭志华教授认为心衰水肿的核心病机为脏气亏虚、气血失和、血水互结,治疗当以补气活血行水为基,补益脏腑为本,主张扶正养心温阳、补肺健脾、益气养血以固本,活血行瘀利水祛其邪。Professor GUO Zhihua integrates the theory of“Inhibited Blood Leading to Water Disease”with the physiological and pathological characteristics of the heart,lungs,and spleen in the clinical treatment of heart failure complicated by refractory edema,emphasizing the principles of seeking treatment from the root cause and holistic differentiation of the Five Zang-Organs.Professor GUO believes that the core pathogenesis of heart failure edema lies in deficiency of visceral qi,disharmony of qi and blood,and binding of blood and water.Treatment should focus on supplementing qi,promoting blood circulation,and eliminating water,while strengthening organs as the foundation.He advocates reinforcing the healthy qi,nurturing the heart and warming yang,supplementing the lungs and fortifying the spleen,replenishing qi and nourishing blood to consolidate the foundation,and promoting blood circulation,removing stasis,and draining water to dispel pathogenic factors.
