数字乳腺断层摄影检查对导管内病变的诊断价值

摘  要:目的探讨导管造影后数字乳腺断层摄影(digital breast tomosynthesis,DBT)检查对伴病理性乳头溢液(pathological nipple discharge,PND)导管内病变的诊断价值。方法收集2021年4月~2022年4月在哈尔滨医科大学附属第一医院诊治并经病理证实的125例伴PND导管内病变患者的临床、病理及影像资料,利用受试者工作特征曲线(receiver operating characteristic curve,ROC曲线),比较导管造影后DBT及全数字化乳腺摄影(full-field digital mammography,FFDM)检查、超声检查的诊断准确性、敏感度、特异度、阳性预测值、阴性预测值及曲线下面积(area under curve,AUC)值。结果DBT检查的诊断准确性、灵敏度、特异度、阳性预测值及阴性预测值均高于FFDM和超声(P<0.05)。DBT的AUC值均高于FFDM及超声,且差异显著(P均<0.05)。结论乳腺导管造影后DBT检查对伴PND导管内病变的诊断效能均高于导管造影后FFDM检查及超声检查,因此,DBT检查可作为伴PND患者的常规检查方法。Objective To explore the diagnostic value of digital breast tomography(DBT)after galactography for intraductal lesions with pathological nipple discharge(PND).Methods The receiver operating characteristic(ROC)curve was used to compare the diagnostic accuracy,sensitivity,specificity,positive predictive value,negative predictive value,and area under curve(AUC)value of DBT-galactography and full-field digital mammography(FFDM)after galactography and ultrasound.Results The diagnostic accuracy,sensitivity,specificity,positive predictive value and negative predictive value of DBT-galactography were higher than those of FFDM and ultrasound(P<0.05).Respectively,the AUC values of DBT were higher than those of FFDM and ultrasound(all P<0.05).Conclusion The diagnostic efficacy of DBT examination after galactography for intraductal lesions with PND is higher than that of FFDM examination and ultrasound examination after galactography,so DBT-galactography can be used as a routine examination method for patients with PND.
