21例左主干闭塞急诊经皮冠状动脉介入治疗患者的临床分析

摘　　要：目的:分析左主干闭塞急诊经皮冠状动脉介入治疗(PCI)患者的临床特点。方法:收集2018年5月至2022年5月于我院接受急诊冠状动脉造影,并证实为左主干急性闭塞患者的临床资料,共21例,均行急诊PCI术,根据存活情况分为存活组(12例)和死亡组(9例),回顾性分析其临床特点及预后。结果:21例左主干闭塞患者病死率为42.85%。死亡组患者的肌酐和NT-proBNP水平高于存活组,发病至开通血管时间长于存活组(P<0.05)。存活组左主干植入支架率与术后冠状动脉血流恢复TIMI3级的比例高于死亡组(P<0.05)。结论:急诊PCI术为目前治疗左主干闭塞的有效手段,但术后病死率仍较高,其预后影响因素多,该病最佳治疗方案还需要进一步研究。Objective:To analyze the clinical characteristics of patients undergoing emergency percutaneous coronary intervention(PCI)for left main artery occlusion.Methods:From May 2018 to May 2022,the clinical data of 21 patients with acute occlusion of the left main artery confirmed by emergency coronary angiography in our hospital were collected.All patients underwent emergency PCI.According to the survival condition,they were divided into survival group(12 cases)and death group(9 cases).Their clinical characteristics and prognosis were retrospectively analyzed.Results:The case fatality of 21 patients with left main artery occlusion was 42.85%.The levels of creatinine and NT-proBNP in the death group were higher than those in the survival group,and the time from onset to vascular opening was longer than that in the survival group(P<0.05).The stent implantation rate and the proportion of postoperative coronary blood flow returning to TIMI perfusion grade 3 in the survival group was higher than that in the death group(P<0.05).Conclusion:Emergency PCI is an effective method for the treatment of left main artery occlusion at present,but its postoperative case fatality is stil high,and there are many factors affecting the prognosis.The optimal treatment of this disease still needs further study.
