摘　　要：目的观察上颈段关节松动术对慢性颞下颌关节紊乱的疗效。方法将60例慢性颞下颌关节紊乱患者随机分为两组,每组30例。观察组采用上颈段关节松动术+颞下颌关节物理治疗;对照组采用上颈段假性关节松动术+颞下颌关节物理治疗。比较两组治疗前后的VAS疼痛评分、最大主动张口度(MMO)以及下颌功能限制量表(JFLS-20)评分变化。结果治疗2周后,两组的VAS疼痛评分、MMO和JFLS-20评分均较治疗前改善(P<0.01),且观察组的改善程度大于对照组(P<0.05)。治疗3个月后,观察组MMO和JFLS-20评分的改善大于对照组(P<0.05或P<0.01)。结论上颈段关节松动术可改善慢性颞下颌关节紊乱患者的MMO和下颌功能,减轻疼痛。Objective To observe the efficacy of upper cervical joint mobilization in the treatment of chronic temporomandibular disorders.Methods Sixty patients with chronic temporomandibular disorders were randomly divided into two groups with 30 cases each.The patients in group A were treated with upper cervical joint mobilization and temporomandibular joint physical therapy and those in group C were treated with upper cervical pseudoarthrosis and temporomandibular joint physical therapy.The changes in VAS pain scores, maximum mouth opening(MMO),and jaw functional limitation scale(JFLS-20) scores before and after treatment were compared between the two groups.Results Compared to before, the VAS pain scores, MMO,and JFLS-20 scores of the two groups after 2 weeks of treatment were all obviously improved(P<0.01),the improvements of which were more in group A than those in group C(P<0.05).After 3 months of treatment, the improvements of the MMO and JFLS-20 scores were better in group A than those in group C(P<0.05 or P<0.01).Conclusion Upper cervical joint mobilization can improve MMO and jaw function, and alleviate pain of patients with chronic temporomandibular disorders.
