黄芪桂枝五物汤加味联合火龙罐治疗2型糖尿病周围神经病变的临床效果观察

摘　　要：目的:观察黄芪桂枝五物汤加味联合火龙罐治疗2型糖尿病周围神经病变(DPN)的临床效果。方法:选取2022年1月—2023年12月玉林市中医医院收治的80例2型DPN患者作为研究对象,按就诊顺序分为观察组和对照组,每组各40例。对照组采用黄芪桂枝五物汤加味治疗,观察组在对照组基础上联合火龙罐治疗,均持续治疗14 d。比较两组临床疗效、治疗前后中医症状积分、血糖水平[空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)]以及神经功能指标[正中神经(MN)与腓总神经(CPN)的运动神经传导速度(MNCV)和感觉神经传导速度(SNCV)]。结果:观察组临床治疗总有效率优于对照组,差异有统计学意义(P<0.05)治疗后,两组中医症状积分、FPG、2hPG、HbA1c水平降低,且观察组低于对照组,差异有统计学意义(P<0.05)两组MN与CPN的MNCV和SNCV显著提升,观察组优于对照组,差异有统计学意义(P<0.05)。结论:黄芪桂枝五物汤加味联合火龙罐治疗DPN疗效显著,可明显缓解DPN患者临床症状,增强降糖效果,并加快各神经传导速度。Objective:To observe the clinical effect of Huangqi Guizhi Wuwu decoction added flavor combined with Fire dragon jar in the treatment of type 2 diabetic peripheral neuropathy(DPN).Method:80 cases of type 2 DPN patients admitted to Yulin Hospital of Traditional Chinese Medicine from January 2022 to December 2023 were selected as the study subjects,and they were divided into an observation group and a control group according to the order of consultation,with 40 cases in each group.The patients in the control group were treated with Huangqi Guizhi Wuwu decoction added flavor,and the patients in the observation group were treated with fire dragon jar treatment on the basis of the control group,and both of them continued to be treated for 14 d.Clinical efficacy,TCM symptom scores,blood glucose levels[fasting blood glucose(FPG),2 h postprandial blood glucose(2 h PG),glycosylated hemoglobin(HbA1c)],and neurological function indexes[motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of the median nerve(MN)and the common peroneal nerve(CPN)]before and after the treatment were compared between the two groups of patients.Result:The total effective rate of clinical treatment of patients in the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the TCM symptom scores,FPG,2 h PG,HbA1c levels of the two groups of patients were reduced,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).The MNCV and SNCV of MN and CPN were significantly elevated in both groups,and the patients in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:The efficacy of Huangqi Guizhi Wuwu decoction added flavor combined with Fire dragon jar in the treatment of DPN is remarkable,which can significantly alleviate the clinical symptoms of DPN patients,enhance the effect of glucose lowering and accelerate t
