小切口骨折撬拔复位结合交锁髓内钉内固定术对跟骨骨折患者的影响

摘　　要：目的:分析小切口骨折撬拔复位结合交锁髓内钉内固定术对跟骨骨折患者的影响。方法:回顾性选取2020年9月—2023年9月德化县医院收治的60例跟骨骨折患者作为研究对象。根据治疗方式不同将其分为对照组和观察组,各30例。对照组接受切开复位内固定术,观察组接受小切口骨折撬拔复位结合交锁髓内钉内固定术。比较两组围手术期指标,术前及术后48 h影像学参数,术前及术后3个月足踝功能及并发症。结果:观察组手术时间、住院时间和骨折愈合时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。术后48 h,两组Bohler角升高,Gissane角、跟骨宽度均降低,观察组Bohler角高于对照组,Gissane角和跟骨宽度均低于对照组,差异有统计学意义(P<0.05)。术后3个月,观察组美国足踝骨科学会踝-后足功能评分(American Orthopedic Foot and Ankle Society,AOFAS)、Maryland足部评分系统(Maryland foot score,MFS)评分均高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:小切口骨折撬拔复位结合交锁髓内钉内固定术利于跟骨骨折患者术后恢复,可改善影像学指标和足踝功能,减少并发症。Objective:To analyze effect of small-incision fracture prying and pulling reset combined with interlocking intramedullary nailing internal fixation surgery on patients with calcaneal fracture.Method:Sixty patients with calcaneal fracture admitted to Dehua County Hospital from September 2020 to September 2023 were retrospectively selected as the study objects.According to different treatment methods,the patients were divided into control group and observation group,30 cases in each group.The control group received open reduction and internal fixation surgery,and the observation group received small-incision fracture prying and pulling reduction combined with interlocking intramedullary nail internal fixation surgery.Perioperative indexes,imaging parameters before and 48 h after surgery,ankle function and before and 3 months after surgery and complications were compared between the two groups.Result:The operation time,hospital stay and fracture healing time in the observation group were shorter than those in the control group,and the intraoperative blood loss volume was less than that in the control group,the differences were statistically significant(P<0.05).At 48 h after surgery,Bohler angle increased,Gissane angle and calcaneal bone width decreased in both groups,Bohler angle in observation group was higher than that in control group,Gissane angle and calcaneal bone width were lower than those in control group,the differences were statistically significant(P<0.05).Three months after surgery,American Orthopedic Foot and Ankle Society(AOFAS)and Maryland foot score(MFS)scores of the observation group were higher than those of the control group,the differences were statistical significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Small-incision fracture prying and pulling reduction combined with interlocking intramedullary nail internal fixation surgery is beneficial to postoperative recover
