痔瘘洗剂熏洗坐浴联合常规治疗湿热下注证混合痔术后患者疗效观察

摘　　要：目的探讨痔瘘洗剂熏洗坐浴联合常规治疗湿热下注证混合痔术后患者的疗效。方法采用随机数字表法将2021年10月—2022年9月在安徽中医药大学第一附属医院接受治疗的60例湿热下注证混合痔术后患者分为A组和B组,各30例。A组给予温水熏洗坐浴和常规换药,B组给予痔瘘洗剂熏洗坐浴及常规换药。两组均持续治疗3~4周。比较两组治疗3~4周后的临床疗效,治疗前、治疗3、7 d后的疼痛评分、水肿症状积分及创面渗液评分,术后住院时间、创面愈合时间、出血消失时间、肛门分泌物消失时间、创面面积愈合率,治疗期间的不良反应。结果治疗3~4周后,与A组比较,B组总有效率更高[93.33%(28/30)vs 70.00%(21/30),P<0.05]。治疗7 d后,两组视觉模拟评分(visual analogue scale,VAS)、水肿症状积分及创面渗液评分均低于治疗前、治疗3 d后,治疗1周后低于治疗前,且治疗3、7 d后,与A组比较,B组更低(P<0.05)。与A组比较,B组出血消失、创面愈合、术后住院及肛门分泌物消失时间均更短,创面面积愈合率更高(P<0.05)。B组和A组治疗期间的不良反应发生率比较,差异无统计学意义[3.33%(1/30)vs 10.00%(3/30),P>0.05]。结论痔瘘洗剂熏洗坐浴能够有效减轻湿热下注证混合痔患者疼痛及水肿状况,减少患者创面分泌物,促进伤口愈合,进一步促进术后恢复,减少患者住院的时间,具有较高的临床疗效,且安全性良好。Objective To explore the curative effect of hemorrhoid fistula lotion fumigation and hip bath combined with routine treatment on patients with mixed hemorrhoids of dampness-heat pouring down syndrome after operation.Methods Sixty patients with mixed hemorrhoids treated in the hospital from October 2021 to September 2022 were divided into the group A and the group B by random number table method,with 30 cases each group.The group A was given warm water hip bath and routine dressing change,while the group B was given hemorrhoid fistula lotion fumigation hip bath and routine dressing change.Both groups were treated for 3-4 weeks.The clinical effects after 3 to 4 weeks of treatment,the scores of including pain,edema symptom and wound exudation before treatment,3 d and 7 d after treatment,the times of hospital stay,wound healing,bleeding disappearance,anal secretion disappearance,rate of wound area healing,and adverse reactions during treatment were compared between two groups.Results After 3 to 4 weeks of treatment,compared with that in the group A,the total effective rate in the group B was higher[93.33%(28/30)vs 70.00%(21/30),P<0.05].After 7 d of treatment,the scores of visual analogue scale(VAS),edema symptom and wound exudation of the two groups were lower than those before and after 3 d of treatment,and the levels after 1 week of treatment were lower than those before treatment,and after 3,7 d of treatment,compared with those in the group A,the score and edema symptom and wound exudation in the group B were lower(P<0.05).Compared with those of the group A,the times of bleeding disappearance,wound healing,hospital stay and anal secretion disappearance in the group B were shorter,and the rate of wound area healing was higher(P<0.05).There was no significant difference of the incidence of adverse reactions between the two groups during the treatment[3.33%(1/30)vs 10.00%(3/30),P>0.05].Conclusion Hemorrhoid fistula lotion could effectively relieve pain and edema in patients with mixed hemorrhoids of dampness-heat pou
