复荣通脉胶囊对糖尿病合并冠心病患者糖脂代谢、血管内皮功能及血小板活化的影响

摘　　要：目的探讨复荣通脉胶囊对糖尿病合并冠心病患者糖脂代谢、血管内皮功能及血小板活化的影响。方法采用随机数字表法将96例糖尿病合并冠心病患者分为对照组与观察组,各48例。对照组给予皮下注射利拉鲁肽注射液治疗,观察组在此基础上给予口服复荣通脉胶囊治疗。连续治疗4周后,评估两组治疗效果,观察两组治疗前后糖脂代谢、血管内皮功能及血小板活化相关指标变化情况,记录两组不良反应发生情况。结果治疗4周后,观察组临床有效率高于对照组(P<0.05)。治疗4周后,两组的空腹血糖(fasting blood glucose,FBG)、餐后2 h血糖(2 h postprandial blood glucose,2 h PG)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)均降低(P<0.05),且观察组低于对照组(P<0.05)。治疗4周后,两组血清一氧化氮(nitric oxide,NO)、血流介导的内皮依赖性舒张功能(flow-mediated dilation,FMD)均升高(P<0.05),且观察组高于对照组(P<0.05);治疗后,两组血清内皮素-1(endothelin-1,ET-1)均降低,且观察组低于对照组(P<0.05)。治疗4周后,两组血小板颗粒膜蛋白CD63、溶酶体膜蛋白CD62p、血小板膜糖蛋白纤维蛋白原受体(platelet activator combined-1,PAC-1)均降低,且观察组低于对照组(P<0.05)。两组治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论在常规西药治疗的基础上联合复荣通脉胶囊有利于提升糖尿病合并冠心病患者治疗效果,其作用机制可能与调节糖脂代谢、改善血管内皮功能及抑制血小板活化相关。Objective To investigate the effect of Furong Tongmai Capsule(复荣通脉胶囊)on glucose and lipid metabolism,vascular endothelial function and platelet activation in patients with diabetes and coronary heart disease.Methods A total of 96 diabetic patients with CHD were divided into control group(48 cases)and observation group(48 cases)by random number table method.The control group was treated by subcutaneous injection of liraglutide,and the observation group was given oral Furong Tongmai Capsule on the basis of the treatment.After 4 weeks of continuous treatment,the treatment effect of the two groups was evaluated.The changes of glucose and lipid metabolism,vascular endothelial function and platelet activation related indicators were observe before and after treatment,and the adverse reactions of the two groups were recorded.Results After 4 weeks of treatment,the clinical response rate was higher than that of the control group(P<0.05).After 4 weeks of treatment,the levels of fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG)and low-density lipoprotein cholesterol(LDL-C)in the two groups decreased(P<0.05),and the levels of the observation group were lower than those of the control group(P<0.05).After 4 weeks of treatment,the levels of serum nitric oxide(NO)and flow-mediated dilation(FMD)in both groups were higher than those in the control group(P<0.05).After treatment,the level of serum endothelin-1(ET-1)in both groups was lower than the control group(P<0.05).After 4 weeks of treatment,the levels of CD63,CD62p and platelet activator combined-1(PAC-1)in both groups were lower than those of the control group(P<0.05).The incidence of adverse reactions between the two groups was compared during treatment(P>0.05).Conclusion On the basis of conventional western medicine treatment,the combination of Furong Tongmai Capsule is conducive to improving the treatment effect of patients with diabetes and coronary heart disease and and its
