完带汤加减结合磷霉素钙颗粒治疗小儿外阴阴道炎(脾虚湿困证)临床观察

摘　　要：目的探讨完带汤加减结合磷霉素钙颗粒治疗小儿外阴阴道炎(脾虚湿困证)的临床疗效。方法选择2021年6月—2022年4月在保定市中医院接受治疗的72例小儿外阴阴道炎患者,采用随机数表法分为试验组和对照组,各36例。对照组给磷霉素钙颗粒口服治疗,试验组给予完带汤加减结合磷霉素钙颗粒口服治疗。比较两组临床疗效、白细胞介素2(interleukin-2,IL-2)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素13(interleukin-13,IL-13)、中医症状积分、临床症状缓解情况及并发症发生情况。结果治疗后,两组总有效率比较,差异有统计学意义(P<0.05);治疗后,两组IL-2、TNF-α、IL-13水平均降低,试验组降低更为明显(P<0.05);治疗后,两组阴道灼热感、外阴瘙痒、头晕及口干尿赤评分水平均降低,试验组降低更为明显(P<0.05);治疗后,试验组外阴瘙痒、外阴疼痛及白带异常改善时间均显著低于对照组(P<0.05);治疗期间不良反应主要有恶心呕吐、头晕、头痛,两组发生率比较,差异无统计学意义(P>0.05)。结论在小儿外阴阴道炎中完带汤加减结合磷霉素钙颗粒治疗有较好的效果,可有效改善患者炎症水平。Objective To study the effect of Modified Wandai Decoction(完带汤加减)combined with Fosfomycin Calcium Granules in the treatment of children with vulvovaginitis(spleen deficiency and dampness syndrome).Methods Seventy-two pediatric patients with vulvovaginitis who received treatment in the hospital from June 2021 to April 2022 were selected and divided into experimental group and control group by random number table method,with 36 cases in each group.The control group was given oral treatment with fosfomycin calcium granules,and the experimental group was given oral treatment with Modified Wandai Decoction combined with Fosfomycin Calcium Granules.The clinical efficacy,interleukin-2(IL-2),tumor necrosis factor-α(TNF-α),interleukin-13(IL-13),TCM symptom scores,clinical symptom relief and complications were compared between the two groups.Results The total effective rate of the two groups was compared and the difference was statistically significant(P<0.05).After treatment,the levels of IL-2,TNF-αand IL-13 in both groups decreased,with the experimental group showing a more significant decrease(P<0.05).After treatment,the levels of vaginal burning sensation,external genital itching,dizziness,dry mouth and red urine score in both groups decreased,with the experimental group showing a more significant decrease(P<0.05).After treatment,the improvement time of vulvar itching,vulvar pain and abnormal vaginal discharge in the experimental group was significantly lower than that in the control group(P<0.05).During the treatment period,the main adverse reactions were nausea,vomiting,dizziness and headache.The incidence rates of the two groups were not significant(P>0.05).Conclusion The combination of Modified Wandai Decoction and Fosfomycin Calcium Granules has a good effect in the treatment of vulvovaginal inflammation in children which can effectively improve the inflammatory level of patients.
