川芎定痛饮加减联合盐酸氟桂利嗪胶囊治疗偏头痛疗效及对脑血流动力学和血清神经递质的影响

摘　　要：目的探讨川芎定痛饮加减联合盐酸氟桂利嗪胶囊治疗偏头痛的疗效和安全性。方法偏头痛患者86例随机分为对照组43例与试验组43例。对照组予以盐酸氟桂利嗪胶囊治疗,试验组予以川芎定痛饮加减联合盐酸氟桂利嗪胶囊治疗,两组均连续治疗4周。观察两组疗效和不良反应,于治疗前和治疗后对患者进行偏头痛中医证候积分、视觉模拟量表(visual analogue scale,VAS)评分和特异性偏头痛生活质量量表(migraine quality of life scale,MSQ)评分,进行脑血流速度[大脑前动脉(anterior cerebral artery,ACA)、大脑后动脉(posterior cerebral artery,PCA)、大脑中动脉(middle cerebral artery,MCA)]和血清神经递质[5-羟色胺(5-hydroxytryptamine,5-HT)、降钙素基因相关肽(calcitonin gene-related peptide,CGRP)、β-内啡肽(β-endorphin,β-EP)]的测定。结果试验组显愈率显著高于对照组(P<0.05),两组总有效率差异无统计学意义(P>0.05)。治疗后,试验组中医证候积分和VAS评分显著低于对照组(P<0.05),MSQ评分显著高于对照组(P<0.05)。治疗后,试验组ACA、PCA、MCA水平和血清CGBP水平均显著低于对照组(P<0.05),5-HT、β-EP水平显著高于对照组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论川芎定痛饮加减联合盐酸氟桂利嗪胶囊治疗偏头痛患者能够更有效缓解患者偏头痛症状,提高疗效,其作用机制可能与调节脑血流速度和血清神经递质表达有关。Objective To investigate the efficacy and safety of Chuanxiong Dingtong Decoction(川芎定痛饮)combined with flunarizine hydrochloride capsules in the treatment of migraine.Methods Eighty-six patients with migraine were randomly divided into control group(43 cases)and experimental group(43 cases).The control group was treated with flunarizine hydrochloride capsules,and the experimental group was treated with Chuanxiong Dingtong Decoction combined with flunarizine hydrochloride capsules.Both groups were treated continuously for 4 weeks.The efficacy and adverse reactions of the two groups were observed.Before and after treatment,migraine TCM syndrome scores,visual analogue scale(VAS)scores and specific migraine quality of life scale(MSQ)scores were performed on patients,and cerebral blood flow velocity[anterior cerebral artery(ACA),posterior cerebral artery(PCA),middle cerebral artery(MCA)]and serum neurotransmitters[5-hydroxytryptamine(5-HT),calcitonin gene-related peptide(CGRP),β-endorphin(β-EP)]were measured.Results The cure rate of the experimental group was significantly higher than that of the control group(P<0.05).After treatment,the TCM syndrome score and VAS score in the experimental group were significantly lower than those in the control group(P<0.05),and the MSQ score was significantly higher than that in the control group(P<0.05).After treatment,the levels of ACA,PCA,MCA and serum CGBP in the experimental group were significantly lower than those in the control group(P<0.05),and the levels of 5-HT andβ-EP were significantly higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Chuanxiong Dingtong Decoction combined with flunarizine hydrochloride capsules in the treatment of migraine patients can more effectively alleviate the symptoms of migraine and improve the curative effect.The mechanism may be related to the regulation of cerebral blood flow velocity and serum neurotransmitte
