苍龟探穴针法联合EDP诊治AECOPD的临床疗效及对肺通气功能、机体炎症的影响

摘　　要：目的探究苍龟探穴针法联合体外膈肌起搏器(external diaphragm pacer,EDP)对慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者的临床疗效及对其肺通气功能、机体炎症的影响。方法选取2022年1月—2022年12月珠海市第五人民医院接收的AECOPD患者90例,采用随机数表法分为观察组和对照组,各45例。对照组患者行EDP治疗,观察组在对照组基础上联合探究苍龟探穴针法治疗。比较两组治疗前后临床症状评分、肺通气功能、机体炎症以及血气指标变化。结果治疗后,观察组患者AECOPD临床症状(咳嗽、咯痰、喘息、肺部啰音)评分均低于对照组(P<0.05),同时最大呼气第一秒呼出的气量容积(forced expiratory volume in 1 second,FEV1)、最大自主通气量(maximum autonomous ventilation volume per minute,MVV)以及用力肺活量(forced vital capacity,FVC)均低于对照组(P<0.05)。治疗后,观察组血清降钙素原(procalcitonin,PCT)及C反应蛋白(C-reactive protein,CRP)浓度低于对照组(P<0.05),外周血白细胞计数(white blood cell,WBC)计数低于对照组(P<0.05)。治疗后,观察组患者PaO_(2)高于对照组(P<0.05),PaCO_(2)低于对照组(P<0.05)。结论苍龟探穴针法联合EDP可有效改善AECOPD的临床症状和通气功能,缓解机体炎症,提高患者的血气功能,值得在临床中推广。Objective To explore the clinical efficacy of Canggui acupoint probe acupuncture combined with external diaphragm pacer(EDP)for patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its effects on ventilation and inflammation.Methods A total of 90 patients with AECOPD admitted to the hospital between January 2022 and December 2022 were selected and divided into observation group and control group by random number table method,with 45 patients in each group.The control group received EDP treatment,and the observation group was combined with Canggui acupoint probe acupuncture treatment on the basis of the control group.The clinical symptom scores,lung ventilation function,body inflammation and blood gas indexes of the two groups were compared before and after treatment.Results After treatment,AECOPD clinical symptoms(cough,sputum,wheezing,pulmonary rales scores)in the observation group were lower than those in the control group(P<0.05),while maximum volume of exhaled air in the first second of exhalation(FEV1),maximum voluntary ventilation(MVV)and forced vital capacity(FVC)in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of serum procalcitonin(PCT)and C-reactive protein(CRP)in the observation group were lower than those in the control group(P<0.05),and peripheral blood white blood cell count(WBC)was lower than that in the control group(P<0.05).After treatment,the value of PaO_(2)in the observation group was higher than that in the control group(P<0.05),and PaCO_(2)was lower than that in the control group(P<0.05).Conclusion Canggui acupoint probe acupuncture combined with EDP can effectively improve the clinical symptoms and ventilation function of AECOPD,relieve the inflammation of the body and improve the blood gas function of patients,which is worthy of clinical promotion.
