小针刀联合药物注射治疗Quinnell分级Ⅱ、Ⅲ期屈指肌腱狭窄性腱鞘炎的随机对照研究

摘　　要：目的采用随机对照研究方式分析小针刀联合药物注射治疗Quinnell分级Ⅱ、Ⅲ期屈指肌腱狭窄性腱鞘炎的临床价值。方法将2021年1月1日—2023年1月1日在芜湖市中医医院针灸二科就诊的120例Quinnell分级Ⅱ、Ⅲ期屈指肌腱狭窄性腱鞘炎患者按就诊顺序随机分为治疗组与对照组,每组60例。对照组采用药物注射治疗,治疗组采用小针刀松解和药物联合治疗。比较两组治疗前、治疗第1、2、3、4周视觉模拟疼痛评分(visual analogue scale,VAS)、Quinnell分级法评分、手指关节活动度以及有效性。结果治疗组脱组3例,对照组脱组5例;与治疗前比较,两组治疗后第1、2、3、4周时点的VAS评分、Quinnell分级评分均显著下降,治疗组第1、2、3、4周时点的VAS评分、Quinnell分级评分显著低于对照组(P<0.05);与治疗前比较,两组治疗后第1、2、3、4周时点的手指关节活动度均显著提高,治疗组第1、2、3、4周时点的手指关节活动度显著高于对照组(P<0.05);治疗组有效率为96.5%(55/57),显著高于对照组的72.7%(40/55)(χ^(2)=6.923,P=0.009)。结论小针刀联合药物注射治疗Quinnell分级Ⅱ、Ⅲ期屈指肌腱狭窄性腱鞘炎能够有效降低患指疼痛感、Quinnell分级评分,提高手指活动度、有效性,临床效果优于单用药物注射治疗。Objective To analyze the clinical value of small needle knife combined with drug injection in the treatment of Quinnell gradeⅡandⅢstenosing tenosynovitis involving the flexor digitorum tendons.Methods A total of 120 patients with Quinnell gradeⅡandⅢstenosing tenosynovitis involving the flexor digitorum tendons who were treated in the Second Department of Acupuncture,Wuhu Hospital of Traditional Chinese Medicine from January 1,2021 to January 1,2023 were randomly assigned to the treatment group and the control group with 60 patients in each group.The control group was treated with drug injection,while the treatment group was treated with small needle knife and drug injection.The visual analog scale(VAS)scores,Quinnell classification scores,range of motion of finger joint and effectiveness were compared between the groups.Results There were 3 cases in the treatment group and 5 cases in the control group lost to follow up.After treatment,the VAS scores and Quinnell classification scores of the two groups decreased significantly.The scores of the treatment group were significantly lower than those of the control group(P<0.05).After treatment,the range of motion of finger joint increased significantly,and the range of motion in the treatment group was significantly larger than that in the control group(P<0.05).The effective rate in treatment group(96.5%,55/57)was significantly higher than that in the control group(72.7%,40/55)(χ^(2)=6.923,P=0.009).Conclusion Small needle knife combined with drug injection for treating Quinnell gradeⅡandⅢstenosing tenosynovitis involving the flexor digitorum tendons can effectively alleviate pain of affected fingers,lower Quinnell classification scores and improve the range of motion of fingers.The clinical effect is better than the treatment with drug injection alone.
